Licensing Specialist observed two staff members that have not completed the 40 hour Introductory Child Care Training courses as of inspection date. LS gave technical assistance from the Child Care Facility Handbook: 4.2.1 Mandated Introductory Training Child care personnel must successfully complete 40 hours of child care training as evidenced by successful completion of competency examinations offered by the Department or its designated representative with a weighted score of 70 or better. 03-01 · Ratio Sufficient
Counselor observed 5 infant children with 1 teacher. The teacher used her walkie talkie to call for assistance. It took the facility about 15 minutes to send second personnel to keep the ratio in compliance. This was resolved by a second staff member coming in the room to help. (4) STAFF-TO-CHILDREN RATIO. (a) Minimum standards for the care of children in a licensed child care facility as established by rule of the department must include: 1. For children from birth through 1 year of age, there must be one child care personnel for every four children. 2. For children 1 year of age or older, but under 2 years of age, there must be one child care personnel for every six children. 3. For children 2 years of age or older, but under 3 years of age, there must be one child care personnel for every 11 children. 4. For children 3 years of age or older, but under 4 years of age, there must be one child care personnel for every 15 children. 5. For children 4 years of age or older, but under 5 years of age, there must be one child care personnel for every 20 children. 6. For children 5 years of age or older, there must be one child care personnel for every 25 children. 7. When children 2 years of age and older are in care, the staff-to-children ratio shall be based on the age group with the largest number of children within the group. (b) This subsection does not apply to nonpublic schools and their integral programs as defined in s. 402.3025 (2)(d)1. Has an industry start date of 12/13/2023. She is currently missing Child Growth and Development, Identifying and Reporting Child Abuse and Neglect. 2. C.J. Has an industry start date of 9/13/22. She is currently missing the Child Growth and Development and Behavioral Observation and Screening course to be completed in order to be in compliance with the School Readiness Preservice requirements. 3. D.L. Has completed her courses bur late. Has an industry date of 11/18/2021, started courses on 02/18/2021 and completed the training on :02/22/2023. 18 Training Requirements 18.1 Pre-service Timeframe All pre-service training requirements listed below must be completed by all program personnel, volunteers and substitutes, each as defined in this handbook, within 90 days of initial employment with any provider participating in the school readiness program. Counselor reviewed 82 children's files. The following file did not have a Florida Certification of Immunization (DH Form 680) on file. Standard was resolved when the director asked the parents for a copy of the child's records at the time of pick up. Child A.D. was enrolled on 6/22/2023. 30 days or more has passed from the date of enrollment. 7.1 Immunization Records The child care facility is responsible for obtaining for each child in care a current, complete and properly executed Florida Certification of Immunization form Part A-1, B, or C, DH 680, which is incorporated by reference in 65C-22.001(7)(o), F.A.C., or the Religious Exemption from Immunization form, DH 681, which is incorporated by reference in 65C22.001(7)(p), F.A.C., from the custodial parent or legal guardian. DH Form 680 and DH Form 681 may be obtained from the local county health department. Specific immunization requirements are included and detailed in the most current edition of the Immunization Guidelines-Florida Schools, Child Care Facilities and Family Day Care Homes as promulgated by the Florida Department of Health. Counselor received a copy of the facility's discipline policy. A complaint was made indicating a child was placed in the restroom area to calm down. Counselor reviewed the Discipline Policy provided. The Discipline policy does not state that staff will use restroom areas as an alternate area or used space for children to calm down. Counselor has determined that the facility failed to follow their own discipline policy handbook. 2.8 Child Discipline B. The child care facility operators, employees, and volunteers must comply with written disciplinary and expulsion policies. The facility was placed on a provisional license for 6 months to obtain an active credentialed director for the facility as of 5/18/2023. Counselor gave technical assistance from the Child Care Facility Handbook: 4.7 Director Credential Section 402.305(2)(g), F.S., requires a child care facility to have a credentialed director. Every applicant for a license to operate a child care facility or a license for a change of ownership of a child care facility must document that the facility director has an active Director Credential prior to issuance of the license.
