Counselor reviewed all 72 active children's Immunization Records. One record was expired. Expiration date was: 02/10/2024. Child's D.O.B.: 01/20/2019 and enrollment date: 07/01/2022. This was resolved when the facility called the parent and advised them that their child was terminated from enrollment. Parent will be allowed to re-enroll once all documents are up to date. 7.1 Immunization Records The child care facility is responsible for obtaining for each child in care a current, complete and properly executed Florida Certification of Immunization form Part A-1, B, or C, DH 680, which is incorporated by reference in 65C-22.001(7)(o), F.A.C., or the Religious Exemption from Immunization form, DH 681, which is incorporated by reference in 65C22.001(7)(p), F.A.C., from the custodial parent or legal guardian. DH Form 680 and DH Form 681 may be obtained from the local county health department. Specific immunization requirements are included and detailed in the most current edition of the Immunization Guidelines-Florida Schools, Child Care Facilities and Family Day Care Homes as promulgated by the Florida Department of Health

Counselor reviewed all 72 actively enrolled children's Student Health Examination records. Three of those records were missing the check mark stating that the child may participate fully in school activities including physical education. One record was missing the date on form. One was missing the Signature of the Physician and the date. Total of Five records were not in compliance with standards. Child 1: Enrollment date: 08/11/2022. D.O.B.: 08/16/2019 Child 2: Enrollment date: 09/01/2022 D.O.B: 09/25/2018 Child 3: Enrollment date: 08/10/2023 D.O.B:05/20/2020 Child 4: Enrollment date: 08/10/2023 D.O.B:02/14/2020 (Missing date on form) Child 5: Enrollment date: 06/16/2023 D.O.B: 08/04/2020 (Missing date and Physician's signature. This was resolved at the time of inspection. Facility reached out to the parents told to pick up their children and can return only when updated records have been provided. 7.2 Student Health Records T he child care facility is responsible for obtaining for each child in care a current, complete and properly executed Student Health Examination form DH 3040, which is incorporated by reference in 65C-22.001(7)(q), F.A.C. and may be obtained from the local county health department, the parent or legal guardian, or a signed statement by authorized professionals that indicate the results of the components of the Student Health Examination form are included in the health examination
