33-09 · Training Requirements
Specialist observed personnel (R.B) in the infant classroom of the facility at time of inspection. As of 7/27/2025, R.B has not completed the Safe Sleep/Shaken Baby Syndrome training. Per the CCF Handbook; 4.2.3 Safe Sleep/ Shaken Baby Syndrome Training All child care personnel who work in a facility that offers care to infants must have training regarding guidance on safe sleep practices, preventing shaken baby syndrome and abusive head trauma; recognition of signs and symptoms of shaken baby syndrome and abusive head trauma; strategies for coping with crying, fussing, or distraught child and the development and vulnerabilities of the brain in infancy in early childhood within 30 days of hire at the facility. For child care personnel to satisfy this requirement the training must be accomplished through one of following methods: Department's Health Safety and Nutrition course, Safe Sleep course, or the Early Learning Floridas Safe Sleep Practices. Documentation of training must be maintained on the Department's training transcript in the child care personnel record. R.B was observed at the facility for a previous inspection on 6/4/25 as a volunteer. It was stated that R.B would be hired once her background screening had cleared so she could be in the classroom by herself. R.B was observed at the center at time of inspection in the classroom alone. Specialist is giving the provider until 8/22/25 to come back into compliance. The training was completed on 8/13/2025, as evidenced by official training transcript

45-02 · Background Screening Documents
Specialist reviewed personnel documentation for R.B who was observed in Classroom B at time of inspection. On 7/16/2025, R.B's background screening was still in review and an eligibility determination had not been made. 5.1 Initial Screening A screening must be conducted as a condition of employment. The employer/owner/operator must review each employment application to assess the relevancy of any issue uncovered by the complete background screening, including any arrest, pending criminal charge, or conviction, and must use this information in employment decisions in accordance with state laws. A. Level 2 screening as outlined in s. 435.04, F.S., is required for all child care personnel and includes a criminal records check (both national and statewide), a sexual predator and sexual offender registry search, and child abuse and neglect history of any state in which an individual resided during the preceding 5 years. All fingerprints must be submitted and processed through the Background Screening Clearinghouse and therefore a Live Scan vendor that is Clearinghouse compatible must be used for submission of fingerprints. 03-01 · Ratio Sufficient
Specialist observed Classroom A with the following ratio; 9 children total; one two-year old, one infant, and 7 one-year olds to two personnel. Because of the infant, the classroom ratio needed a total of 3 personnel to be in compliance. Per the CCF Handbook, under Ratios; 2.3.1 Mixed Age Groups A. In groups of mixed age ranges, where children under one year of age are included, one child care personnel must be responsible for no more than four children of any age group, at all times. B. In groups of mixed age ranges, where children one year of age but under two years of age are included, one child care personnel must be responsible for no more than six children of any age group, at all times. Additionally in Supervision 2.4 2.4.2 Nap time Supervision During nap time, child care personnel must be within sight and hearing of all the children. All other child care personnel required to meet the staff-to-child ratio must be within the building on the same floor and available to be summoned if needed to ensure the safety of the children. Nap time supervision does not apply to children up to 24 months of age, who must have direct supervision at all times. To bring the class back into compliance, the two-year old was moved to Classroom B which had a total of 12 three-year olds, totaling thirteen children to one personnel when the child was moved. The classroom ratio after the move was as such; Classroom A with 8 children; 7 one-year olds and 1 infant to two personnel. Counselor observed pre-k classroom with 10 children and no staff member. Counselor observed 2 children records missing student health enrollment records after 30 days of enrollment. The provider is to submit the records to bring facility into compliance. 7.2 Student Health Records The child care facility is responsible for obtaining for each child in care a current, complete and properly executed Student Health Examination form DH 3040, which is incorporated by reference in 65C-22.001(7)(q), F.A.C. and may be obtained from the local county health department, the parent or legal guardian, or a signed statement by authorized professionals that indicate the results of the components of the Student Health Examination form are included in the health examination. A. The Student Health Examination shall be completed by a person given statutory authority to perform health examinations. B. The Student Health Examination or the signed statement is valid for two years from the date the physical was performed. An up-to-date version must be on file for as long as the child is enrolled at the facility. C. If the custodial parents or legal guardians fail to provide the documentation required above within 30 days of enrollment, the facility shall not allow the child to remain in the program
