During the routine inspection, the Licensing Specialist observed that the provider did not have a current Background Screening (BGS) clearance. Documentation on file indicated that the providers BGS eligibility expired on April 2, 2026. The Licensing Specialist provided the following TA (Technical Assistance): Section 4.1 of the Florida Family Day Care Home/Large Family Child Care Home Handbook and section 402.313(3), Florida Statutes (F.S.) state that all childcare personnel, including the owner/operator, must complete background screening prior to employment and must be re-screened at least every five (5) years to maintain eligible status in the DCF Clearinghouse. Corrective Action Plan: The provider agreed to complete the required Background Screening (BGS) re-screening within 21 business days (three weeks) and will submit verification of updated BGS eligibility to the Licensing Specialist via email upon completion. The provider acknowledged that her substitute caregiver will be responsible for the supervision of currently enrolled children until the BGS re-screening process is completed and eligibility is confirmed. The provider further agreed to notify the Licensing Specialist via email prior to the enrollment of any additional children. The BGS re-screening will be completed no later than May 14, 2026. The provider also stated that she will update her information in the DCF Clearinghouse to include her current email address to ensure timely receipt of BGS renewal notifications

FSC observed provider to have (7) children in care at time of inspection. The Florida Statutes state the following. 2.2 Licensed Capacity 2.2.1 Family Day Care Home A family day care home may provide care for one of the following groups of children, which shall include household children under 13 years of age: A. A maximum of four children from birth to 12 months of age. B. A maximum of three children from birth to 12 months of age, and other children, for a maximum total of six children. C. A maximum of six preschool children if all are older than 12 months of age. D. A maximum of 10 children if no more than 5 are preschool age and, of those 5, no more than 2 are under 12 months of age. Corrective action: This standard was found to be non-compliance, because the most preschoolers the provider can have at one time are 6. The child was picked up by parent therefore the standard is completed

FSC observed that provider giving transportation to enrolled children. Provider did not inform FSC that transportation was being given at her FDCH and did not complete vehicle inspection as required. The following TA was provided from the FDCH Handbook 2.4.4, A All vehicles regularly used to transport children must be inspected annually by a mechanic to ensure that they are in proper working order. Documentation by the mechanic must be maintained in the vehicle. Corrective action: Provider was given TA and will not be transporting children until all transportation needs are completed therefore the standard was completed at the time of inspection

06-01 · Background Screening Requirements FDCH/LFCCH Handbook
FSC observed volunteer (JM) did not have a required background screening. The Florida FDCH Handbook states the following. 4.1 Initial Screening Operators, household members (adults and children 12 to 17 years of age), substitutes, volunteers and Large Family Child Care Home employees must have a level 2 background screening clearance from the Department prior to obtaining a license, residing in the home, employment, or volunteering unsupervised with children. The employer/owner/operator must review each employment application to assess the relevancy of any issue uncovered by the complete background screening, including any arrest, pending criminal charge, or conviction, and must use this information in employment decisions in accordance with state laws. A. Level 2 screening as outlined in s 435.04, F.S., is required for all child care personnel and includes a criminal records check (both national and statewide), a sexual predator and sexual offender registry search, and child abuse and neglect history of any state in which an individual resided during the preceding 5 years. All fingerprints must be submitted and processed through the Background Screening Clearinghouse and therefore a LiveScan vendor that is Clearinghouse compatible must be used for submission of fingerprints. Household members aged 12 to 17 years must complete a Juvenile Records Check with FDLE. Providers may use the public portal to request certified State of Florida criminal history juvenile record checks from this link: https://shield.fdle.state.fl.us/shield/app/orisearch. Corrective Action: JM will not return to work until BGS is completed as required by the Florida Statutes therefore the standard was completed at the time of inspection

07-02 · Staff Training FDCH/LFCCH Handbook
FSC observed substitutes/volunteer (JM) transcript did not have the required training hours for a FDCH. FSC gave TA from the Florida FDCH Handbook as follows. 5.1.1. Operators and Substitutes Prior to licensure and prior to caring for children, all home operators, substitutes for the operator, and substitutes working more than 40 hours per month on average over a 6-month period must: A. Successfully complete the Departments 30-clock-hour Family Child Care Home training, as evidenced by successful completion of a competency-based examination(s) offered by the Department or its designated representative with a weighted score of 70 or better. Operators who successfully completed the mandatory 30-clock-hour Family Child Care Home training prior to January 1, 2004, are not required to fulfill the competency examination requirement. Documentation of course completion may either be a single-Family Child-Care Home certificate or certificates for the five individual training courses which total 30-clock-hours of training: Family Child Care Home Rules and Regulations; Health, Safety and Nutrition; Identifying and Reporting Child Abuse and Neglect; Child Growth and Development; and Behavioral Observation and Screening. All certificates must be reflected on the training transcript. Corrective action: The substitute will no longer care for children until requirements are met therefore the standard was completed at the time of inspection
