12-02 · Technical assistance provided. Per Provider the sink is stopped up right now but will be getting repaired. In the meantime, the children will wash their hands in the sink in the dining room area. 3 Physical Environment Children are much more susceptible to the adverse effects of environmental factors, materials and toxicants. 12-02 · Facility Environment
The 3 year old classroom had areas of paint peeling off of the walls. 3.1 General Health and Safety Requirements A. All child care facilities must be clean, in good repair, free from health and safety hazards and from evidence of, or presence of, vermin infestation. Indoor play areas must be inspected daily for basic health and safety and documented on a daily inspection log. Outdoor play areas must be inspected daily for basic health and safety. Any problems must be corrected before the play area is used by children. Documentation of the indoor play area inspection must be maintained for 12 months. B. It is the responsibility of the director/operator to ensure all areas and equipment of the facility are free from fire hazards, such as lint build up in heating and air vents, filters, exhaust fans, ceiling fans, and dryer vents. 26-02 · Meal and Snack Menus
The facility did not have documentation of a menu. Director stated that she did not know she still needed a menu once she is no longer on the food program. H. Meal and snack menus must be planned, written, dated, and posted at the beginning of each week in an easily seen place, accessible to the parents/guardians. Any menu substitutions must be noted on the menu. A generalized menu of possible snack choices for programs that receive food donations is acceptable. All meals and snacks prepared outside of the facilitys kitchen or designated food preparation area, such as catered food, must be listed along with the source. Daily meal and snack menus must be maintained for a minimum of 12 months for licensing purposes. Operators who participate in the USDA Food Program must keep menus in accordance with the Department of Health and USDA requirements

33-03 · Training Requirements
Staff member listed on supplemental was hired on 04/05/23, did not start beginning training within 90 days of hired. Training was not started until October 21, 2023. This was corrected at the time of inspection, training has been started. 4.1 Beginning Training Child care personnel including volunteers who work 10 hours or more per month must begin training within 90 days of employment in the child care industry and successfully complete Departments training within 12 months from the date training begins. Training taken prior to employment in the child care industry does not constitute begin training. The begin training timeframes begin at the time of employment in the child care industry. Counselor observed that the playground fencing was not attached to the fencing posts or other fence panels in several places and the fence panels had gaps larger than 3 1/2 inches. Corrective action will be for the fencing to be connected to all posts and panels and there to be no gaps between any of the fence posts or panels by the due date. (HANDBOOK) The outdoor play area must have adequate fencing or walls a minimum of 4 feet in height. Counselor observed a knife on the kitchen counter during the walk through of the facility. A school age child had walked into the kitchen before the Counselor and then she returned to her class. Corrective action was to remove the knife from the counter at time of inspection. (Handbook) B. 33-01 · Training Requirements
Counselor found that a staff member has not completed her required 40 Hour training and was in charge of a class of children. The facility was previously cited on 12/13/2022 for the same staff member. At that time, the staff member was removed from the classroom and reassigned to "Other Personnel" to bring the facility back into compliance. Corrective action for this citation is for the staff member to complete the required 40 hr training by the due date. Until the corrective action is completed, the staff member can not be in the classroom alone with children. A fully trained staff member must be present in the classroom any time the untrained staff member is in the classroom. No breaks, No lunch coverage, No bathroom breaks, No covering alone for absent teachers. The staff member can work in areas not caring for children. 04-10 · Supervision
Per provider BL and staff/employee CW, a child was left without supervision inside the childcare facility causing a child to be injured. Family Service Counselor (FSC) provided TA from the Handbook in Section 2.4.1, B that states the following: Childcare personnel must be assigned to provide direct supervision to a specific group of children and be with that group of children at all times. Children must never be left without childcare personnel supervision inside or outside the facility, in a vehicle, or at a field trip location. Corrective Action: The action was immediately corrected by childcare staff returning to her classroom where the 1yr old children were located; therefore, the children were able to have direct supervision as required in the classroom
