03-01 · Ratio Sufficient
Licensing Specialist observed the following ratio during inspection: ------School Age Children- 1 staff; 13 children ------School Age children 1 staff; 14 children -------Mixed age group of 4 & 5 year old- 1 staff; 15 children -------Mixed age group of 2,3,4 year old & School Age Children- 1 staff; 13 children (There were 5- 2 year old, 4- 3 year old, 3- 4 year old & 1- School Ager) --------2 additional staff members Licensing Specialist observed the Mixed age group of 2,3,4 year old & School Age Children to out of ratio during inspection. LS observed a total of 13 children on the outside playground with 1 staff member. LS observed 5- 2 year old, 4- 3 year old, 3- 4 year old & 1 School Ager present. The provider moved 1- school age child from the mixed age group of 2,3,4, year old & school age to the school age group during inspection. The provider moved 1- 4 year old from the mixed age group of 2.3.4 year old & school age to the 4 & 5 year old during inspection. The ratio in the mixed age group of 2,3,4 & school age, came back into compliance with ratio during inspection with the following ratio: ------School Age- 1 staff; 14 children ------School Age- 1 staff: 14 children -------Mixed age group of 4 & 5 year old- 1 staff; 16 children ------Mixed age group of 2,3,4 year old- 1 staff; 11 children (There were 5- 2 year old, 4- 3 year old & 2- 4 year old) ------2 additional staff members LS gave technical assistance from the Florida Statutes: 402.305 Licensing standards; child care facilities. (4) STAFF-TO-CHILDREN RATIO. (a) Minimum standards for the care of children in a licensed child care facility as established by rule of the department must include: 1. For children from birth through 1 year of age, there must be one child care personnel for every four children. 2. For children 1 year of age or older, but under 2 years of age, there must be one child care personnel for every six children. 3. For children 2 years of age or older, but under 3 years of age, there must be one child care personnel for every 11 children. 4. For children 3 years of age or older, but under 4 years of age, there must be one child care personnel for every 15 children. 5. For children 4 years of age or older, but under 5 years of age, there must be one child care personnel for every 20 children. 6. For children 5 years of age or older, there must be one child care personnel for every 25 children. 7. When children 2 years of age and older are in care, the staff-to- children ratio shall be based on the age group with the largest number of children within the group. Corrective Action: The provider moved 1- school age child from the mixed age group of 2,3,4, year old & school age to the school age group during inspection. The provider moved 1- 4 year old from the mixed age group of 2.3.4 year old & school age to the 4 & 5 year old during inspection. The ratio was put back into compliance at the time of inspection with the mixed age group of 2,3,4 year old having 1 staff; 11 children. Therefore the standard was completed at the time of inspection

Licensing Specialist observed 15 transportation logs from 2/13/2025 to 5/23/25 that did not include one or more of the following: each child's name, the signature of a second child care personnel or person(s) authorized by the provider to verify the transportation log and that all children have arrived safely and left the vehicle, the date the driver and/or second child care personnel completed the visual sweep of the transportation vehicle. LS gave technical assistance from the Child Care Facility Handbook: 2.5.2 Transportation Log Corrective Action: Provider is to submit to Licensing Specialist via e-mail a written statement from the provider, driver(s) & any secondary personnel that is authorized to conduct secondary visual sweeps of the transportation vehicle, that they understand and will maintain the rules and regulations from CCF Handbook, Section 2.5.2

06-01 · Driver's License, Physician Certification
Licensing Specialist observed one driver that did not have an annual physical examination which grants medical approval to drive. LS observed the one driver to be transportation children since at least March 2025. The driver stated to the LS during inspection that they have not completed an annual physical medically approving them to drive. LS gave technical assistance from the Child Care Facility Handbook: 2.5.1 Driver Requirements The driver of any vehicle used by a child care program to provide transportation must have the following: A. A valid Florida drivers license including the proper endorsement; B. An annual physical examination which grants medical approval to drive, and valid certificate(s) of course completion for first aid training and pediatric cardiopulmonary resuscitation (CPR) procedures. 45-02 · Background Screening Documents
Licensing Specialist observed one staff member with employment date at the child care facility of 5/21/2025 that had a background screening clearance letter eligibility status of 10/9/2023. The staff member was a volunteer at a child care facility in October 2023 to December 2023 and has not been present in the child care industry in between December 2023 and 5/21/2025. LS gave technical assistance from the Child Care Facility Handbook: 7.4.1 Background Screening Documents Background screening documentation must be maintained for all child care personnel as defined by Section 402.302(3), F.S., which includes household members if the facility is located in or adjacent to the home of the operator. Background screening documentation must be on-site and available for the licensing authority to review. C. A copy of the eligible results, for the Level 2 screening, generated from the Clearinghouse must be on record for each personnel. If awaiting Out-of-State criminal history results, a copy of the DCF email informing of the individuals eligibility for a provisional hire status must be in the personnel file. Corrective Action: Provider is to initiate a resubmission for the one staff member's background screening. Provider is to submit documentation to Licensing Specialist via e-mail that the resubmission has been initiated for the one staff member
