33-01 · Training Requirements
At the time of inspection, the LS reviewed the transcript of V.A. who began employment at the facility on 12/20/24. V.A. was previously employed in the childcare industry in March 2010. V.A. has not completed the required childcare training and was observed at the time of inspection alone in a classroom. The staff member's training should have been completed June 2011. 4.2 Training Requirements 4.2.1 Mandated Introductory Training Childcare personnel must successfully complete 40 hours of childcare training as evidenced by successful completion of competency examinations offered by the Department or its designated representative with a weighted score of 70 or better. Childcare personnel who successfully completed the mandatory 40-hour Introductory Child Care Training prior to January 1, 2004, are not required to fulfill the competency examination requirement. A. Part I Courses (30 Hours) Childcare personnel must complete all of the following: • ? Child Care Facility Rules and Regulations • ? Health, Safety and Nutrition • ? Identifying and Reporting Child Abuse and Neglect • ? Child Growth and Development • ? Behavioral Observation and Screening B. Part II Courses (10 Hours) Childcare personnel must also complete 10 hours of the following Part II courses: • ? Special Needs Appropriate Practices (10 hours), or • ? Understanding Developmentally Appropriate Practices (5 hours) and one • of the following courses: • 1. Infant and Toddler Appropriate Practices (5 hours) • 2. Preschool Appropriate Practices (5 hours) • 3. School- Age-Appropriate Practices (5 hours) Staff must not be alone in a classroom until successful completion of childcare training. 13-03 · Toxic Substances, Hazardous Materials and Weapons
At the time of inspection, the licensing specialist observed a closet ajar and unlocked. In the closet the licensing specialist observed chemicals such as a can of paint potentially accessible to children. Per the provider the closet door was left ajar due to a child hiding in the closet prior to the licensing specialist arriving to conduct the routine inspection. 3.2 Toxic/Hazardous Material/Firearms/Weapons A. All areas and surfaces accessible to children must be free from toxic substances, bio contaminants, and hazardous materials/equipment/tools, including power tools, plastic bags, matches, candles, lighters, etc. B. All potentially harmful items, including cleaning supplies, flammable products, poisonous, toxic, and hazardous materials, must be labeled and used according to manufacturers recommendation. These items, as well as knives, sharp tools, and other potentially dangerous hazards, must be stored in a locked area or must be inaccessible and out of a childs reach at all times. The provider locked and closed the utility closet door. Hazardous materials were no longer accessible to children as the door is now locked

21-11 · Restrooms and Bath Facilities
At the time of reinspection, the back cover of the toilet was had not been repaired by the corrective action date of 4/24/2024. 3.7 Restrooms and Bath Facilities I. Each toilet and sink must be maintained in good operating condition, cleaned and sanitized or disinfected as needed, but at least once per day. Sinks utilized for teeth brushing activities/procedures by the children must be sanitized prior to the activity occurring. The provider will replace/repair the toilet back cover. During nap time the counselor observed 21 students ages 2 to 4 years of old being supervised by 1 teacher; where majority of the children were 3 years of age.
