Licensing Specialist observed 3 staff members child abuse and neglect forms that were signed after employment date at the facility. Licensing Specialist gave technical assistance from the School Age Handbook: 7.4 Personnel Records C. A signed CF-FSP Form 5337, Child Abuse &Neglect Reporting Requirements form. CF-FSP Form 5337 is incorporated by reference in paragraph in 65C-22.001(7)(l), F.A.C., must be signed on or before hire date and annually thereafter by all child care personnel. Licensing Specialist observed that the 3 staff members did not sign the Attestation of Good Moral Character prior to employment. Licensing Specialist gave technical assistance from the School Age Handbook: 7.4.1 Background Screening Documents B. CF Form 1649A, Child Care Attestation of Good Moral Character, must be completed for all child care personnel at the time of initial screening or upon change in employers. CF Form 1649A is incorporated in reference by 65C22.001(7)(a), F.A.C., and may be obtained from the Departments website at www.myflfamilies/childcare.com . Corrective Action: Two staff members signed the Attestation of Good Moral Character as of inspection date. The third staff member did not have the Attestation of Good Moral Character signed or present during inspection. The third staff member is to sign the Attestation of Good Moral Character. 21-08 · Fire Drills
Counselor observed during inspection that a fire drill was not documented for the facility for the months of February or March 2024. Counselor gave technical assistance during inspection from the School Age Handbook: 3.8.4 Fire Drills C. A current attendance record and parent contact information must accompany child care personnel out of the building during a drill or actual evacuation and be used to account for all children. When the facilitys approved alarm system is activated, all adults and children must evacuate the facility. The operator must maintain a written record of the fire drills showing the date, number of children and child care personnel in attendance, evacuation route used, and time taken for all individuals to evacuate the premises. Each fire drill record must be maintained for a minimum of 12 months from the date of the fire drill. The fire drills conducted must include, at a minimum: 1. One fire drill using an alternate evacuation route, and 2. One drill in the presence of and at the request of the licensing authority in coordination with the operator or designee. 28-03 · Training Requirements SACCF Handbook
Counselor observed one staff member with child care industry start date as of 4/24/2023 and an employment date at the facility of 9/11/2023 that had not begun the introductory training as of inspection date. Counselor gave technical assistance from the School Age Handbook: 4.1 Beginning Training Child care personnel must begin training within 90 days of employment and successfully complete the Departments training within 12 months from the date training begins. Training taken prior to employment in the child care industry does not constitute begin training. The begin training timeframe begins at the time of employment in the child care industry. Training completion may not exceed 15 months from the date of employment in the child care industry in any licensed Florida child care facility. This may be accomplished by classroom attendance in a Department-approved training course, acquiring an educational exemption from a Department-approved training course, beginning a Department-approved online child care training course, or by receiving results from a Department-approved competency examination. The child care program is responsible for obtaining training documentation from child care personnel. Corrective Action: Staff member is to begin the introductory training. Provider is to submit to counselor via e-mail the staff member's DCF Training Transcript showing that the staff began the introductory training
