05-02 · Transportation
Per the Director the children enrolled no longer go on nature walks or field trips; however annually around graduation season, the children will go to the Church by foot accompanied by staff. No transportation logs were maintained for these trips off campus. The LS reviewed documents of a field trip conducted in January 2025 to Christ the King Catholic School. Per the provider she was unaware documentation of field trips are required. 2.5 Transportation Child care providers must comply with minimum health and safety standards to ensure the well-being of children in their care being transported. For the purposes of this section, transportation pertains to travel by foot or in a vehicle that is owned, operated or regularly used by the child care program, and vehicles used to provide transportation through a contract or agreement with an outside entity. Prior to transporting children, the facility must be approved by the Department to offer transportation services. Transportation services will be approved if the conditions set forth in parts 2.5.1, 2.5.4., 2.5.5., and 4.2.6., of this Handbook are met. Pursuant to the Child Safety Alarm bill in 2021, all vehicles used to transport children by child care facilities and large family child care homes must be equipped with a reliable alarm system which prompts the driver to inspect the vehicle for children before exiting the vehicle. The provider must add transportation to services offered as travel by foot is transporting the children in care to a local outside of the childcare facility. When transporting the provider will maintain transportation logs and any other required documentation for a minimum of 12 months and follow DCF standards for transportation including completing transportation training prior to transporting the children in care

At the time of inspection, the LS observed medications in classrooms that were expired. Per the provider some of the medications belonged to children no longer enrolled. 6.5 Medication Childcare programs are not required to give medication; however, if a program chooses to do so, it must comply with the following requirements: H. Medication that has expired or that is no longer being dispensed must be returned to the custodial parent or legal guardian or discarded. The provider discarded the medications belonging to disenrolled children into the trash. Some medications were sent home from the children currently enrolled

41-01 · Student Health and Records
At the time of inspection, the LS reviewed all eighty-five (85) enrolled students files; and three (3) enrolled student files were missing student health exam forms/physicals Form 3040. 7.2 Student Health Records The child care facility is responsible for obtaining for each child in care a current, complete and properly executed Student Health Examination form DH 3040, which is incorporated by reference in 65C-22.001(7)(q), F.A.C. and may be obtained from the local county health department, the parent or legal guardian, or a signed statement by authorized professionals that indicate the results of the components of the Student Health Examination form are included in the health examination. C. If the custodial parents or legal guardians fail to provide the documentation required above within 30 days of enrollment, the facility shall not allow the child to remain in the program. The provider must obtain an up-to-date copy of the Student Health Examination/DH (Form 3040) prior to accepting the students into care

22-05 · Fire Drills
Counselor observed fire extinguisher has not been properly maintained with being serviced and retagged. Counselor give technical assistance: Fire extinguishers with a minimum rating of 2A:10BC must be properly installed, serviced and maintained with current inspection tags at all times. Director has until June 19 to correct fire extinguishers
