At the time of inspection, counselor observed 6 children in the infant room with 1 staff member. There were 3 (one year old children) and 3 (infants). The teacher to child ratio in the infant room is 1 staff member to 4 infants. This was completed and brought to compliance at the time of inspection by removing the 3 (one year old) toddlers to their designated classroom. 2.3 Ratios The staff-to-child ratio, as established in Section 402.305(4), F.S., is based on primary responsibility for the direct supervision of children and applies at all times while children are in care.2.3.1 Mixed Age Groups A. 04-01 · Supervision
Counselor arrived at 8:15am. Counselor observed the infant room out of ratio, with three one-year olds and three infants to one staff member. The one-year-old teacher was not in her classroom, once she realized DCF was in the building, counselor observed her take the 3 one year old children from the infant room and move them to the toddler room. At this time, there were children being dropped off, causing the toddler teacher to ask the infant teacher to stand between both doorways, so she could have access to the front door. This was corrected once the director arrived and was able to receive the incoming children and taking them to their designated areas. 2.4 Supervision B. Child care personnel must be assigned to provide direct supervision to a specific group of children and be with that group of children at all times. Children must never be left without child care personnel supervision inside or outside the facility, in a vehicle, or at a field trip location. C. Supervision standards apply at all times away from the child care facility, including during field trips, outdoor play, and when picking up or dropping off children at designated locations, such as bus stops, school, or a childs home. For school-age children, personnel must know where the children are and what they are doing at all times and capable of responding to emergencies including when children are separated from their group

13-02 · Toxic Substances, Hazardous Materials and Weapons
Counselor entered the diaper /restroom area in the toddler 2's classroom and observed several cleaning agents accessible to children. 3.2 Toxic/Hazardous Material/Firearms/Weapons B. All potentially harmful items, including cleaning supplies, flammable products, poisonous, toxic, and hazardous materials, must be labeled and used according to manufacturers recommendation. These items, as well as knives, sharp tools, and other potentially dangerous hazards, must be stored in a locked area or must be inaccessible and out of a childs reach at all times. This was corrected and brought back to compliance after the director and counselor did a walk through together and made sure the items were locked or put away from the reach of children. Counselor viewed 7 jugs of juice stored directly on the floor. In the front lobby as you enter the facility, counselor witnessed baby food and cans of infant formula stored directly on the floor underneath a table. 3.9.2 Food Storage Proper storage of food is essential to prevent food contamination, as well as, insect and rodent infestation. Correct handling and storage of all food is a key component in preventing food-borne illnesses. To prevent bacteria growth, cold food must be kept at or below 41 degrees Fahrenheit and hot foods at or above 135 degrees Fahrenheit. Facilities choosing to prepare food must have a designated space for food storage within the designated food preparation area or in a room not calculated as part of indoor floor space, and in an area not used for diapering. Off-site food storage is permissible only if the site of storage is a licensed child care facility under the same ownership that includes a food preparation area that meets licensing standards. A. The parent/guardian of a child who has not received the age-appropriate immunizations prior to enrollment and who does not have documented medical or religious exemptions from routine childhood immunizations must provide documentation of a scheduled appointment or arrangement to receive immunizations. Providers must include a general statement in parent handbook/policies to inform parents/guardians, at time of enrollment, that some children in care may not have current immunizations. C. School-aged children attending public or non-public schools are not required to have student health examination and immunization records on file at the child care facility as such records are on file at the school where the child is enrolled. D. If the custodial parents or legal guardians need assistance concerning these requirements, the facility shall refer them to the Department of Health or to the childs physician. E. Medical records in this section are the property of the custodial parent or legal guardian and must be returned to them when the child withdraws from the facility. The medical records are transferable if the child attends another facility. F. Technical Assistance given from the handbook: 4.1 Beginning Training Child care personnel including volunteers who work 10 hours or more per month must begin training within 90 days of employment in the child care industry and successfully complete Departments training within 12 months from the date training begins.
