05-06 · Transportation
Specialist was observing as the first bus arrived from picking up the afterschool children. Specialist observed the children exit the bus and were brought inside, but no second adult performed a physical inspection of the vehicle as the bus left for it's next destination. Specialist notes that the bus picks up from one school, drops off, then leaves again to pick up from another school before staying at the facility. Specialist continued to observe drop of two more times, and a second sweep was conducted. Specialist spoke with the director on the correct procedures for the vehicle check, and what steps can be made in the future to assure it is not repeated. Specialist assisted in the latter dismissal procedures to assure all children had exited the vehicle. Per the CCF Handbook, 2.5.2 Transportation Log F. Upon arrival at the destination by vehicle, the driver of the vehicle must: 1. Mark each child off the log as the child departs the vehicle; 2. Conduct a physical inspection and visual sweep of the vehicle interior to ensure that no child is left in the vehicle; and 3. Record, sign, and date the transportation log immediately, verifying that all children were accounted for, and that the visual sweep was conducted. 4. Ensure that a second child care personnel conducts a second visual sweep, signs and dates the transportation log verifying that all children were accounted for, and that the log is complete. 5. If the provider contracts with an outside entity to provide transportation, then the provider must assign a child care personnel to perform the duties of the driver outlined above in numbers 1-3. G. Upon arrival at the destination by vehicle, a second and different child care personnel must: 1. Conduct a physical inspection and visual sweep of the vehicle to ensure that no child is left in the vehicle; and 2. Sign, date and record the transportation log immediately, verifying that all children were accounted for, and that the log is complete. Specialist cooperated with the director on arranging a staff training to refresh with the transportation procedures. Director will forward to specialist confirmation that the personnel had attended before 2/26/2025

Counselor revied medications in Classroom B and Classroom G that had incomplete records. In Classroom B, The prescription medication records for Z.W were not marked off to show that the staff had administered the medication according to the prescriptions instructions, which were prescribed as twice daily. According to the personnel in that classroom, Z.W no longer needs that medication. In Classroom G, A.B did not have all of the information on the sheet filled out (name of medication, medication directions). According to personnel in that classroom, A.B has not yet started that medication

07-03 · Technical assistance was provided for maintaining documentation of annual vehicle inspection. No annual inspection was on file for two of the buses utilized at the facility. Appointments were made for the inspections at time of renewal inspection, and proof of inspection will be sent to the licensing counselor as soon as available

33-03 · Training Requirements
Counselor observed that personnel listed on the supplemental did not begin introductory training within 90 days of employment. Completed at time of inspection since personnel began training after 90 days of employment. Provider will ensure that future personnel begin the training within 90 days of employment. Counselor provided the following technical assistance: Child care personnel including volunteers who work 10 hours or more per month must begin training within 90 days of employment in the child care industry and successfully complete Departments training within 12 months from the date training begins. Training taken prior to employment in the child care industry does not constitute begin training. The begin training timeframes begin at the time of employment in the child care industry. Training completion may not exceed 15 months from the date of employment in the child care industry in any licensed Florida child care facility. This may be accomplished by classroom attendance in a Department-approved training course, acquiring an educational exemption from a Department-approved training course, beginning a Department-approved online childcare training course, or by receiving results from a Department-approved competency examination. The childcare program is responsible for obtaining training documentation from childcare personnel
