The transportation log was missing the signature of a second child care personnel or person(s) authorized by the provider to verify the transportation log and that all children have arrived safely and left the vehicle (if applicable). 2.5.2 Transportation Log A. 04-04 · Supervision
Licensing Specialist has determined that there was not adequate supervision in the outdoor playground. A child was left alone in the outdoor playground for over 4 minutes unsupervised. One teacher ran inside as the children were running into the room. She wanted to make sure they were being supervised inside. A second teacher was left alone trying to count and continue collecting the children to line up and prepare to transition inside. While inside the teachers failed to notice a child was missing. A teacher from another room knocked on the classroom door and told them that there was a child left outside. This was resolved when the child was taken inside and was found by another teacher. Current Director now makes a visual sweep of the playground areas as soon as classes transition inside to ensure that no one gets left behind. 2.4 Supervision 2.4.1 General Supervision Requirements A. 39-01 · Accident/ Incident Notification and Documentation
Childcare personnel witnessed a child fall off the slide and cry. Childcare personnel located ice and placed it on the child's injured side (torso). Parent was never notified, called or given a report informing them of injury. Child returned to school 2 days later with a bruised torso and parent questioning what happened and requesting a report to be documented. This was resolved after the childcare personnel who treated the child wrote a report for the parent. 6.3 Emergency Telephone Numbers A. Emergency service telephone numbers must be posted on or near all telephones, including 911, ambulance, fire, police, poison control center, Florida Abuse Hotline, and the county public health department. Additionally, the address and directions to the facility must be posted with descriptions of major intersections and local landmarks. B. For life threatening injuries, the provider shall call 911 and then notify the parent. C. Custodial parents or legal guardians must be notified immediately if the event of any serious illness, accident, injury or emergency to their child, and their specific instructions regarding action to be taken under such circumstances must be obtained and followed. D. If the custodial parent or legal guardian cannot be reached, the facility owner or director will contact those persons designated by the custodial parent or legal guardian to be contacted under those circumstances and must follow any written instructions provided by the custodial parent or legal guardian on the enrollment form

Childcare personnel had 2 separate incidents in which they failed to notify the parents or legal guardians that an accident/ and or incident occurred. First incident, a child, (K.L.), was left behind for 4 minutes alone in the outdoor play area. Second incident, a child, (B.B.), fell off the slide area on the playground causing some bruising on their torso. Facility failed to notify parents. Parents were not immediately notified. These 2 separate incidents were resolved by the time of inspection. Both parties received documentation a few days after. 6.4 Accident/Incident Notification A. 04-04 · Supervision
The facility is out of compliance with outdoor supervision as 4- 1 year old children were able to leave the playground by pushing on an unlocked gate and were able to get as far as the parking lot. Staff admitted to the children getting out of the gate and were observed by the kitchen staff who saw the children and ran out to get the children back. Staff admitted that the maintenance man assisted with returning the children back to their classroom. One staff admitted that she did not see the children leave as she was tying another child's shoe. Both teachers admitted to not checking the gate to assure that it was locked. A safety plan is to be derived by the facility to assure that something of this nature will not occur again. The facility is to conduct an all staff meeting and each staff member is to read the safety plan, sign it and a copy of the safety plan is to be placed in each staff members file folder indicating that they have read and understand the safety plan. The facility is also to discuss proper supervision while on the playground. Each staff member is to position themselves to be able to see all angles on the playground as well as move around to assure that all children are in view. A copy of the meeting notes showing signature of each staff member in attendance is to be provided to the counselor as corrective action. 2.4 Supervision 2.4.1 General Supervision Requirements A. 12-07 · Facility Environment
The facility is out of compliance with a safety deficiency at the facility as the staff failed to assure that the gate on the playground was secured and locked. Because of this four(4) 1 year old were able to push on the gate and leave the playground without being noticed by the two staff that were on the playground. The children were able to make it to the parking lot before being noticed by the kitchen staff who brought the children back along with the maintenance man who was sitting in the parking lot. Both teachers admitting to not checking the gate. A safety plan is to be derived by the facility to assure that something of this nature will not occur again. The facility is to conduct an all staff meeting and each staff member is to read the safety plan, sign it and a copy of the safety plan is to be placed in each staff members file folder indicating that they have read and understand the safety plan. 3 Physical Environment Children are much more susceptible to the adverse effects of environmental factors, materials and toxicants. It is imperative that the child care environment is conducive to the healthy development of children. 3.1 General Health and Safety Requirements A. All child care facilities must be clean, in good repair, free from health and safety hazards and from evidence of, or presence of, vermin infestation. Indoor play areas must be inspected daily for basic health and safety and documented on a daily inspection log. Outdoor play areas must be inspected daily for basic health and safety. Any problems must be corrected before the play area is used by children. Documentation of the indoor play area inspection must be maintained for 12 months
