The slide on the playground is broken (cracked) on both slides posing a scraping or pinching hazard. Provider is closed March 21st- 26th. Technical Assistance given from handbook: 3.12 Outdoor Equipment A. A child care facility must provide and maintain enough usable equipment suitable to each childs age and development and of a quantity for each to be involved in activities. B. All playground equipment must be securely anchored, unless portable or stationary by design, in good repair, maintained in safe and sanitary condition, and placed to ensure safe use by the children. FSC observed knives and sharp objects in a drawer that was in an open area that was not locked, and kids had access to. All items were moved to a higher area where children couldn't reach. 24-02 · Food Preparation Area
• The infant room has one sink and they have been using the one sink to make bottles and washing hands in the same sink. Provider is going to have parents make the bottles at home and bring them in already made. Provider will send a letter to parents letting them know how to prepare bottles. Provider will send FSC copy of letter. • Technical Assistance given from the handbook: • 3.9.1 Food Preparation Area A food preparation area is a designated room, such as a kitchen, or a designated space in a facility not normally used or accessible to the children in daily operations for indoor play, classroom, work or nap spaces, and not included when calculating usable indoor floor space. A. A food preparation area is required for facilities that choose to prepare food in a manner consistent with the definition of preparation of food. Specific requirements for the food preparation area include:1. 24-18 · Food Preparation Area
FSC observed 6 gallons of milk stored in the refrigerator dates are March 18 & March 19th. Provider stated she just received the food earlier today. Provider is exchanging the milk for fresh milk. Provider also has milk that is in date in another refrigerator and will use that milk. Borden's Milk is replacing the milk by the end of date. Provider needs to email FSC to show where the milk was exchanged. Technical Assistance from handbook: 3.9.2 Food Storage Proper storage of food is essential to prevent food contamination, as well as, insect and rodent infestation. Correct handling and storage of all food is a key component in preventing food-borne illnesses. To prevent bacteria growth, cold food must be kept at or below 41 degrees Fahrenheit and hot foods at or above 135 degrees Fahrenheit. Facilities choosing to prepare food must have a designated space for food storage within the designated food preparation area or in a room not calculated as part of indoor floor space, and in an area not used for diapering. Off-site food storage is permissible only if the site of storage is a licensed child care facility under the same ownership that includes a food preparation area that meets licensing standards. A. Food containers, such as cans, plastic containers, boxes and bags must be stored above the floor on clean surfaces protected from splash and other contamination. B. Stored food must be consumed or discarded on or before the expiration dates listed by the manufacturer. C. Poisonous/toxic chemicals or cleaning products must be stored separately from food. Products must not be stored on shelves above food preparation areas and/or food products intended for human consumption, unless placed in bins that are impermeable. D. 40-22 · Medication
FSC observed a portable nebulizer in the one-year-old room. Inside the nebulizer was a clear liquid that went up halfway inside the nebulizer. Provider states that the mother of the child brings the medication in for an emergency but that the child does not use it here. The teacher stated that he uses it daily. Provider is to have a medication form filled out with proper dosing and times. Technical Assistance given from handbook:6.5 Medication Child care programs are not required to give medication; however, if a program chooses to do so, it must comply with the following requirements: A. The child care program must have written authorization from the custodial parent or legal guardian to give prescription and non-prescription medications. This authorization must be dated and signed by the custodial parent or legal guardian and contain the childs name; the name of the medication to be given; and date, time and amount of the correct dosage to be given. Prescription and non-prescription medications that are used on an as needed basis require the parent/ legal guardian to provide additional documentation on the authorization form to describe symptoms that would require the medication to be given. The child care provider must never administer a medication that is prescribed for one child to another child

03-01 · Ratio Sufficient
Counselor observed the following ratio during inspection: ~Mixed age group of 2, 3, 4 and 5 years old- 1 staff; 20 children (There were 2- 2 years old, 3- 3 years old, 10- 4 years old and 5- 5 years old) ~2 years old- 1 staff; 9 children ~Mixed age group of Infants and 1 years old- 1 staff; 4 children (There were 1- Infant and 3- 1 years old) ~Mixed age group of 1 and 2 years old- 1 staff; 9 children (There were 1-1 year old and 8- 2 years old) Counselor was initially stated information by mixed age group of 1 and 2 years old staff member, that 1 child was 1 years old. The staff member in the mixed age group of Infants and 1 years old then stated that the child was 2 years old. Counselor spoke with on site in charge manager that stated the child she believed the child was 2 years old. Counselor reviewed the child's enrollment form and observed that the child was 1 years old according to their date of birth. Counselor gave technical assistance from the Florida Statutes, s.402.305(4), F.S. 402.305 Licensing standards; child care facilities. (4) STAFF-TO-CHILDREN RATIO. (a) Minimum standards for the care of children in a licensed child care facility as established by rule of the department must include: 1. For children from birth through 1 year of age, there must be one child care personnel for every four children. 2. For children 1 year of age or older, but under 2 years of age, there must be one child care personnel for every six children. 3. For children 2 years of age or older, but under 3 years of age, there must be one child care personnel for every 11 children. 4. For children 3 years of age or older, but under 4 years of age, there must be one child care personnel for every 15 children. 5. For children 4 years of age or older, but under 5 years of age, there must be one child care personnel for every 20 children. 6. For children 5 years of age or older, there must be one child care personnel for every 25 children. 7. When children 2 years of age and older are in care, the staff-to-children ratio shall be based on the age group with the largest number of children within the group. Corrective Action: During inspection, the on site, in charge manager, moved 3- 2 year old's from the mixed age group of 1 and 2 years old to the mixed age group of the 20 children ages 2, 3, 4 and 5 years old classroom during inspection and naptime. The mixed age group of 1 and 2 years old had 1 staff; 6 children ( 1- 1 years old and 5-2 years old), and the mixed age group of 2,3,4 and 5 years old had a total of 23 children (5- 2 years old, 3- 3 years old, 4- 10 years old and 5- 5 years old) during naptime. Therefore, the standard was completed at time of inspection

31-03 · Indoor Equipment
Counselor observed in the Infant/ 1 year old room, a diaper changing table piece of furniture, that had a top surface made of plastic, that was cracked from the left bottom corner to the right bottom corner. Counselor observed the diaper changing area after the staff member changed a child. Counselor observed silver duct tape over the top of most the broken plastic surface, however counselor observed the cracked plastic area to separate when touch and to move inward towards the wooden furniture structure underneath the plastic. Counselor gave technical assistance during monthly check of indoor equipment requirements during inspection. Counselor gave technical assistance during inspection from the Child Care Facility Handbook: 3.11 Indoor Equipment Equipment, materials, furnishings, and play areas should be sturdy, safe, and in good repair. Provider should monitor the Consumer Product Safety Commission (CPSC) recommendations for use of equipment. Walls, ceilings, floors, furnishings, equipment, toys, and other surfaces should be suitable to the location and the users. They should be maintained in good repair, free from visible soil and clean. Equipment and furnishings must be arranged to allow direct supervision of children by child care personnel from all areas within a room/play area. Bookshelves, televisions, and other heavy furnishings must be stable and secured as to not pose a threat of falling over. In efforts to prevent children from climbing to dangerous heights or enabling them to reach hazardous items, chairs and other furnishings must be placed away from cabinets and shelves. The program should make accommodations to the program environment and schedule so that children with special needs may participate. Corrective Action: Provider moved the diaper changing table to be located in between two classrooms, Infants/ 1 year old and 1/ 2 year old classroom during inspection. Provider is to remove the broken diaper changing furniture and submit a picture to counselor via e-mail
