40-06 · Technical assistance was provided to the teacher regarding documenting the administration of medication as well as returning medication that isn't being used
Counselor reviewed the prescription and nonprescription medication the facility currently holds. The prescription medication for I.O has on the prescription label to administer twice daily. On the permission slip signed by the parent, "to be given" time is written as "after being wiped down after playground". No record of medication given was noted on the form, and the medication was initially started 3/6/24. When the counselor asked if they had administered the creme at all, T.W stated she had not and she was told it was an as needed basis. C. Prescription and non-prescription medication brought to the child care facility by the custodial parent or legal guardian must be in the original container. Prescription medication must have a label stating the name and contact information of the physician and/or pharmacy, childs name, name of the medication, and medication directions. All prescription and non-prescription medication must be dispensed according to written directions on the prescription label or printed manufacturers label and maintained at the appropriate temperature. Counselor observed 4 student files (V.B, G.K, K.K, B.J) that had missing immunization records. 7.1 Immunization Records B. If the custodial parents or legal guardians fail to provide the documentation required above within 30 days of enrollment, the facility shall not allow the child to remain in the program. The parent/guardian of a child who has not received the age-appropriate immunizations prior to enrollment and who does not have documented medical or religious exemptions from routine childhood immunizations must provide documentation of a scheduled appointment or arrangement to receive immunizations. Providers must include a general statement in parent handbook/policies to inform parents/guardians, at time of enrollment, that some children in care may not have current immunizations
