10-03 · Field Trip Permission
Counselor observed 4 children's field trip permission forms that did grant the children's parental permission to participate in each field trip. Counselor observed during inspection that 3 children attended 1 to 2 field trips with the facility that the parents did not grant permission for. Counselor observed 1 child's field trip permission form that had parental initials by each field trip, however the parent did not sign the written statement at the bottom of the field trip form. Counselor gave technical assistance during inspection from the Child Care Facility Handbook: 2.7 Field Trip Activities C. Written parental permission must be obtained in the form of a general or event-specific permission slip. 40-12 · Medication
Counselor observed one child's Albuterol inhaler that did not include the updated doctor's note with the increase in the amount and dosage of the medication. Counselor could only observe a prescription during inspection that stated 1 puff every 4 to 6 hours as needed, however counselor observed that staff were administrating 2 puffs every 4 hours as needed. Counselor received a doctor's note on 4/15/2024 via e-mail, prior to submitting inspection to provider for signature, that matched the information located on the medication form. Counselor observed that the doctor's note stated that the note is valid since 1/2024. Counselor gave technical assistance during inspection from the Child Care Facility Handbook: 6.5 Medication Child care programs are not required to give medication; however, if a program chooses to do so, it must comply with the following requirements: F. The facility must maintain a record for each child receiving any medications that documents the full name of the child, the name of the medication, the date and time the medication was given, the amount and dosage, and the name and signature of the person who gave the medication. This record must be initialed or signed by the program personnel who gave the medication. The record must be maintained for a minimum of 12 months after the last day the child received the dosage. Corrective Action: Counselor received a doctor's note on 4/15/2024 via e-mail, prior to submitting inspection to provider for signature, that matched the information located on the medication form. 33-07 · Training Requirements
Counselor observed 5 staff members that did not complete the 10 hours of in-service during the fiscal year July 1, 2022 to June 30, 2023 during inspection. Counselor gave technical assistance from the Child Care Facility Handbook: 4.2.7 Annual In-Service Training Upon successful completion of the 40-hour introductory training requirements, child care personnel must complete a minimum of 10 clock- hours or one CEU of in-service training annually during the states fiscal year beginning July 1 and ending June 30. Corrective Action: Staff members are to complete the 10 hours of in-service hours. Provider is to submit the in-service form with documentation showing completion of the 10 hours of in-service. ****Please note that the in-service hours utilized to complete the corrective action may not be utilized for the current fiscal year too (July 1, 2023 to June 30, 2024)

Counselor observed that the facility utilized an outside transportation source to transportation the children on field trips on 6/7/2023 and 6/9/2023. The facility utilized 2 drivers that had expired annual physical examination which grants medical approval to drive and 1 driver did not have current First Aid/ CPR on file during inspection to review. Provider no longer utilizes the outside transportation system to transport. Counselor gave technical assistance from the Child Care Facility Handbook: 7.4 Personnel Records Records must be maintained and kept current on all child care personnel, as defined by Section 402.302(3), F.S. These records shall be on-site, available for review by the licensing authority and must include: E. For the driver only: Florida drivers license and driver physical examination documentation. A copy of the drivers license and the physician certification, or another form containing the same elements of the physician certification, granting medical approval to operate the vehicle, and valid certificate(s) of course completion for first aid training, pediatric cardiopulmonary resuscitation (CPR) procedures, and background screening must also be maintained in the drivers personnel file. 32-03 · Outdoor Equipment
Counselor observed broken/cracked plastic PVC pipes that were present on two playgrounds and the rock climber on the playground has a hole with cracks that is duct taped. Counselor gave technical assistance from the Child Care Facility Handbook: 3.12 Outdoor Equipment B. All playground equipment must be securely anchored, unless portable or stationary by design, in good repair, maintained in safe and sanitary condition, and placed to ensure safe use by the children. Maintenance must include inspections conducted every month of all supports above and below the ground and of all connectors and moving parts. Documentation of maintenance inspections must be maintained for 12 months. Corrective Action: Provider is to repair the areas and submit photographic evidence to counselor via e-mail

Counselor could not observe the unknown driver's valid Florida driver's license, an annual physical examination which grants medical approval to drive, valid certificate of course completion for first aid training and pediatric cardiopulmonary resuscitation during inspection, due to the information not being present at the facility. The driver's name from the outside source, Rock Transportation, was unknown by the provider during inspection. Counselor could not determine during inspection if the transportation vehicle utilized by the facility on 3/16/2023 had an annual inspection for the transportation vehicle utilized due to no documentation being present at the facility during inspection . Counselor gave technical assistance from the Child Care Facility Handbook: 2.5.4 Vehicle Requirements For the purpose of this section, vehicles refer to those owned, operated or regularly used by the child care program, and vehicles that provide transportation through a contract or agreement with an outside entity. Parents/guardians personal vehicles used for transporting during field trips are excluded from meeting the requirements of this sub-section. A. All vehicles regularly used to transport children must be inspected annually by a mechanic to ensure that they are in proper working order. Documentation by the mechanic must be maintained in the vehicle. 30-03 · Diapering
Counselors observed sippy cups in the one year old room being washed after lunch in the handwashing sink. The sippy cups are brought from home with water, the water is poured out before lunch, and milk is put in all of the sippy cups. After lunch the sippy cups are washed in the handwashing sink with hand soap and water from the handwashing sink is placed in the sippy cups. The sippy cups were removed from the classroom during inspection and taken to the kitchen for proper washing cycle. Counselor gave technical assistance from the Child Care Facility Handbook during inspection: 3.10.2 Diapering Requirements C. Handwashing sinks must not be used for food service preparation, dishwashing, or food clean up. Corrective Action: Provider is to provide a written statement of the facility's plan of action regarding proper washing requirements for the sippy cups. Counselor observed an epi pen present for 1 child that was not in the original container and did not have the prescription present during inspection. Counselor gave technical assistance from the Child Care Facility Handbook during inspection: 6.5 Medication Child care programs are not required to give medication; however, if a program chooses to do so, it must comply with the following requirements: C. Prescription and non-prescription medication brought to the child care facility by the custodial parent or legal guardian must be in the original container. Prescription medication must have a label stating the name and contact information of the physician and/or pharmacy, childs name, name of the medication, and medication directions. All prescription and non-prescription medication must be dispensed according to written directions on the prescription label or printed manufacturers label and maintained at the appropriate temperature.
