Licensing Specialist observed 13 transportation logs during inspection, from 9/23/2024 to 11/26/2024 that did not have all required transportation log components on the transportation form records. The transportation logs were missing the following required documentation: No date driver conducted sweep, missing children departure time off of vehicle, no date of transportation occurred, departure time written with no childs name or childs arrival time on the vehicle, secondary sweep date conducted two days after transportation date occurred, no secondary sweep date written, no secondary sweep conducted, full date transportation occurred not written. Licensing Specialist gave technical assistance from the Child Care Facility Handbook: 2.5.2 Transportation Log Corrective Action: Provider is to submit to Licensing Specialist the transportation logs from 12/9/2024 to 12/13/2024 via e-mail

17-07 · Outdoor Play Area/Fencing
Licensing Specialist observed one gate on the playground that was not at ground level during inspection. Licensing Specialist gave technical assistance during inspection from the Child Care Facility Handbook: 3.5 Outdoor Play Area F. The outdoor play area must have adequate fencing or walls a minimum of 4 feet in height. Fencing, including gates, must be continuous and must not have gaps or opening larger than 3 inches that would allow children to exit the outdoor play area. The base of the fence must remain at ground level and be free from erosion or build up to prevent inside and outside access by children or animals. These areas must have at least two exits, with at least one being remote from the buildings. If an outdoor play area was approved for usage by the Department prior to October 25, 2017, no new exits are required to be added to meet this standard. However, if outdoor play area fencing is changed then the standard would apply, and two exits must be provided. Corrective Action: Provider is to repair gate, maintaining the requirement of the gate/fence being at ground level. Provider is to submit a picture to Licensing Specialist via e-mail showing documenting that the gate/fence is at ground level

33-01 · Training Requirements
Counselor observed a child care personnel with an industry date of Mar 10, 2022, who had not completed the 40-hour Introductory Child Care Training requirement. The facility has been given until 9/25/2023 to come back into compliance by providing documentation of training completion. Technical Assistance from Child Care Facility Handbook: Mandated Introductory Training Child care personnel must successfully complete 40 hours of child care training as evidenced by successful completion of competency examinations offered by the Department or its designated representative with a weighted score of 70 or better. Child care personnel including volunteers who work 10 hours or more per month must begin training within 90 days of employment in the child care industry and successfully complete Departments training within 12 months from the date training begins. Training taken prior to employment in the child care industry does not constitute begin training. The begin training timeframes begin at the time of employment in the child care industry. Training completion may not exceed 15 months from the date of employment in the child care industry in any licensed Florida child care facility. This may be accomplished by classroom attendance in a Department-approved training course, acquiring an educational exemption from a Department-approved training course, beginning a Department-approved online child care training course, or by receiving results from a Department-approved competency examination.
