Licensing Specialist observed that the girls and the boy's bathroom near the kitchen and dining area, both bathrooms have low water pressure when flushing the toilets and lots of water on both bathroom floors due to toilets running over. Per Provider maintenance was called to repair both bathrooms. Children are using another bathroom within the center. 3.7 Restrooms and Bath Facilities I. Each toilet and sink must be maintained in good operating condition, cleaned and sanitized or disinfected as needed, but at least once per day. Licensing Specialist observed that the light in the girl's restroom is out due to the bulb blown. Per Provider the maintenance person need's a ladder that will reach the high ceiling to change the bulb and they will be calling the city for a ladder that will reach. 3 Physical Environment Children are much more susceptible to the adverse effects of environmental factors, materials and toxicants. It is imperative that the childcare environment is conducive to the healthy development of children. A. All childcare facilities must be clean, in good repair, free from health and safety hazards and from evidence of, or presence of, vermin infestation. Indoor play areas must be inspected daily for basic health and safety and documented on a daily inspection log. Outdoor play areas must be inspected daily for basic health and safety. Any problems must be corrected before the play area is used by children. Documentation of the indoor play area inspection must be maintained for 12 months

04-05 · Supervision
Naptime supervision was inadequate as counselor observed during onsite inspection 9- 2 year old's in a classroom by themselves. The teacher who was watching 14- 3 year old children was in her classroom. The teacher stated that she was supposed to be watching the two classrooms during naptime while the teacher was at lunch. However, the teacher was sitting in her classroom and not standing in between the classrooms to assure the safety of the children. The item was corrected at the time of inspection as another teacher was brought into the classroom to supervise the classroom of 9- 2 year old, while the teacher went back to her classroom. 2.4.2 Nap time Supervision During nap time, child care personnel must be within sight and hearing of all the children. All other child care personnel required to meet the staff-to-child ratio must be within the building on the same floor and available to be summoned if needed to ensure the safety of the children. Refer to the supplemental for the staff member who has been in the industry since 2018, and has not completed the required training in order to work in the child care industry. Staff member must complete the training in order to continue work in industry. 4.2 Training Requirements 4.2.1 Mandated Introductory Training Child care personnel must successfully complete 40 hours of child care training as evidenced by successful completion of competency examinations offered by the Department or its designated representative with a weighted score of 70 or better.
