Counselor observed the outdoor play area to be full of trash, peeling paint, debris, roots sticking out of the grounds. Provider has been given until 1/19/2024 to bring back to compliance. 3.1 General Health and Safety Requirements A. Counselor observed the food preparation area missing signs for the handwashing sink only, rinse, wash and sanitize. The facility also did not have when and how to wash your hands posters or instructions by the handwashing stations or diaper changing stations. Counselor asked the provider to print and post instructions on how and when to wash your hands. This was resolved at the time of inspection. 3.10 Sanitation 3.10.1 Handwashing Child care personnel and children must wash their hands thoroughly with soap and running water, dry, and follow personal hygiene procedures for themselves and while assisting others. Counselor observed a staff member (M.F.), did not complete the introductory training within the 90 days of employment in the child care industry. This has been resolved a day after the 90 day time frame. 4.1 Beginning Training Child care personnel including volunteers who work 10 hours or more per month must begin training within 90 days of employment in the child care industry and successfully complete Departments training within 12 months from the date training begins. Counselor asked the infant room for their daily attendance log. Teacher stated she did not have one. The facility's director said she had a clip board with all the students signed in and out. Each classroom did not have access to their own daily attendance. There was no way for the teachers to track when their children arrived or departed. Upon investigation the counselor observed that the facility was out of ratio in the infant room as their was 5 infants with 1 staff member. The item was corrected at the time of investigation as a parent came and picked up one child leaving the ratio to 4- infants with 1 staff. The counselor also observed another class out of ratio which had a mixed group of 8 children which had 6- 1 year old and 2- 2 year old with 1 staff member. The item was corrected at the time as the 2 year old children where moved to another classroom leaving the ratio 6- 1 year old with 1 staff.
