Counselor observed dead roaches inside two cabinets in the kitchen. The cabinets are located at the bottom, one right to the dish washing sink and right to the stove. Provider presented proof of pest control and cleaned the surfaces to remove dead roaches. This violation was corrected at the time of inspection. Licensing specialist observed a blue water table outside the playground in need of cleaning. The water inside the water table was not fresh as it had collected sediment at the bottom of the container and live larvae. Provider emptied the water table and sanitized it. It was determined by another agency with the department that a child was physically injured while at the childcare facility. None of the teachers are aware of how the child suffered the injuries. The child is no longer attending the facility. From the CCF Handbook Section 2.4: Child care personnel must be assigned to provide direct supervision to a specific group of children and be with that group of children at all times. Children must never be left without child care personnel supervision inside or outside the facility, in a vehicle, or at a field trip location

The facility did not complete an accident/incident report after the parent reported the injuries on the child. The child is no longer attending the childcare. From the CCF Handbook, Section 6.3: A. Counselor observed (3) one-year-old children in a mix age classroom with two-year-olds. Counselor provider technical assistance regarding the mix age classroom. A classroom with one-year-olds should follow the one-year-old ratio of 1:6. A classroom which has two- year-olds and older, the ratio to follow is the age-group with the most children. To resolve this issue director stepped into the classroom until children were picked up by parents. Director also hired a new staff member as of 4/12/2024

14-04 · Lighting, Temperature, and Ventilation
Counselor observed the infant room feeling extremely hot. Counselor utilized portable atmosphere thermometer to measure the temperature of the room, which read 90 Degrees Fahrenheit. This was the only room utilized by children that felt unreasonably hot due to the AC not working and the number of windows in the room. The provider moved the infants into the toddler room and adjusted the ratio to comply with the infant ratio. Per provider, someone is going to come and fix the AC but on the mean time the infants will not utilize the room. The provider has until 7/13/2023 to bring this standard back in to compliance by having someone fix the AC so the room maintains a temperature between 65 degrees and 82 degrees Fahrenheit at all times.
