Licensing Specialist observed two separate half tires locate on the playground for climbing purposes that had multiple areas on the tire that were broken with cracks and/or holes that posed a low potential for harm to children. Counselor observed one staff member with employment date at the facility of 2/1/2024, and a child care industry start date of 9/13/2016 that had a background screening clearance eligibility letter on file for 1/31/2022. Counselor observed per 5 year employment history check conducted by provider, that the one staff member's last date in the child care industry was on 12/21/2022 until employment date at the facility on 2/1/2024. Therefore, the one staff member had a 90 day break in-service with their background screening clearance letter. Counselor gave technical assistance during inspection from the Child Care Facility Handbook: 5.2 Re-Screening A screening conducted under this rule is valid for five years, at which time a re-screen must be conducted in the same manner as the initial screening. Counselor observed all 125 enrolled children's records records during inspection. Counselor observed the 113 immunization records during inspection that were on file. Counselor observed during inspection that 12 children's immunization records were not on file for review or had expired. Counselor gave technical assistance from the Child Care Facility Handbook: 7.1 Immunization Records The child care facility is responsible for obtaining for each child in care a current, complete and properly executed Florida Certification of Immunization form Part A-1, B, or C, DH 680, which is incorporated by reference in 65C-22.001(7)(o), F.A.C., or the Religious Exemption from Immunization form, DH 681, which is incorporated by reference in 65C- 22.001(7)(p), F.A.C., from the custodial parent or legal guardian. DH Form 680 and DH Form 681 may be obtained from the local county health department. Specific immunization requirements are included and detailed in the most current edition of the Immunization Guidelines-Florida Schools, Child Care Facilities and Family Day Care Homes as promulgated by the Florida Department of Health. One staff member moved one child as counselor was entering the 3 year old classroom during inspection. The staff member moved the one child to the Mixed age group of 2 and 3 years old during inspection, therefore the facility was in ratio at time of inspection. Counselor gave technical assistance from the Florida Statutes: s.402.305(4), F.S. 402.305 Licensing standards; child care facilities. (4) STAFF-TO-CHILDREN RATIO. (a) Minimum standards for the care of children in a licensed child care facility as established by rule of the department must include: 1. For children from birth through 1 year of age, there must be one child care personnel for every four children. 2. For children 1 year of age or older, but under 2 years of age, there must be one child care personnel for every six children. 3. For children 2 years of age or older, but under 3 years of age, there must be one child care personnel for every 11 children. 4. For children 3 years of age or older, but under 4 years of age, there must be one child care personnel for every 15 children. 5. For children 4 years of age or older, but under 5 years of age, there must be one child care personnel for every 20 children. 6. For children 5 years of age or older, there must be one child care personnel for every 25 children. 7. When children 2 years of age and older are in care, the staff-to-children ratio shall be based on the age group with the largest number of children within the group. (b) This subsection does not apply to nonpublic schools and their integral programs as defined in s. 402.3025(2)(d)1. Counselor observed a one year old classroom that did not utilize their handwashing sink. The staff members stated that they utilize hand sanitizer and do not wash their hands. Counselor observed a 2 year old classroom that has a portable sink that did not have any water in the tank below. Counselor observed one staff member change a child's diaper and she did not wash her hands or the child's hands afterwards. The staff member went to change another child's diaper prior to washing her hands from the previous child's diaper change. Counselor observed the situation and gave technical assistance. The staff member stated that there was not any water in the portable sink the entire morning. The provider came to the classroom and filled the water during inspection. Counselor gave technical assistance from the Child Care Facility Handbook: 3.10.1 Handwashing Child care personnel and children must wash their hands thoroughly with soap and running water, dry, and follow personal hygiene procedures for themselves and while assisting others. Examples of activities when handwashing is required include but are not limited to: before and after eating, immediately following outdoor play, after toileting, following the use of any cleaners or toxic chemicals, before and after administering medication, and during food preparation and snack distribution. The child care program must have written authorization from the custodial parent or legal guardian to give prescription and non- prescription medications. Counselor observed two bottles during inspection that had the child's first name, however the child's last name was missing on the two bottles. Counselor gave technical assistance during inspection from the Child Care Facility Handbook: 3.9.5 Food Handling B. Bottles and sippy cups brought from home shall be individually labeled with the childs first and last name and shall be returned to the custodial parent or legal guardian daily. 33-13 · Training Requirements
Counselor observed one staff member with child care industry start date and employment date at the facility of 9/6/2022 start school readiness training on 9/12/2022. As of inspection date, staff member has not completed the school readiness training. Counselor gave technical assistance from the School Readiness for licensed, licensed exempt and school age based center programs Handbook: 18.1 Pre-service Timeframe All pre-service training requirements listed below must be completed by all program personnel, volunteers and substitutes, each as defined in this handbook, within 90 days of initial employment with any provider participating in the school readiness program. This timeframe does not start over if personnel change employment to another school readiness provider within this 90 days. Personnel who have not completed all pre-service training requirements may not be allowed any unsupervised contact with or care of children in a school readiness program. Corrective Action: Staff member is to complete the school readiness training and submit to counselor via e-mail documentation of training completion from transcript or training certificates

Counselor observed a medication that was administered to a child on 1/17/2023. The prescription stated that the child was to take 1.5 mL twice a day by oral route as directed for 30 days. Counselor observed the medication form that stated the incorrect name of the medication and the dosage to be given stated 2 mL. Counselor observed that the staff member gave the child the medication on 1/17/2023 at 10:00 AM for 2 mL, however the prescription for the medication stated the dosage to be 1.5 mL. Counselor gave technical assistance from the Child Care Facility Handbook: 6.5 Medication C. Prescription and non-prescription medication brought to the child care facility by the custodial parent or legal guardian must be in the original container. Prescription medication must have a label stating the name and contact information of the physician and/or pharmacy, childs name, name of the medication, and medication directions. All prescription and non-prescription medication must be dispensed according to written directions on the prescription label or printed manufacturers label and maintained at the appropriate temperature. Corrective Action: Provider immediately removed the medication from the classroom during inspection. Provider received a doctor's note during inspection with the 2mL listed as the dosage. Provider returned the medication to the parent during inspection and stated to them that they may not bring the medication back to the facility. Therefore, standard was completed during time of inspection

Staff member administered medication to a child and counselor did not observe documentation of education on proper administration procedures on file during inspection. Counselor gave technical assistance from the Child Care Facility Handbook: 6.5 Medication I. Prior to administering medication to children, child care personnel responsible for administering medication must be educated on proper administration procedures. Written documentation must be maintained in the personnel file that child care personnel administering medication have been educated on proper administration procedures. Corrective Action: All staff members are to sign their understanding of maintaining CCF Handbook, Section 6.5, I. Provider is to submit to counselor via e- mail the documentation of all staff members signatures
