05-04 · Technical assistance provided to the director: Child Care Facility Handbook Section, 2.5.2. A state, " A log must be maintained for all children being transported in a vehicle or on foot away from and/or to the premises of the childcare facility. The log must be retained on file at the facility for a minimum of 12 months and available for review by the licensing authority. The log must include: 1. Each childs name, 2. The date and time of departure, 3. Time of arrival at the destination, 4. The signature of the driver (or in the case of travelling on foot, the signature of the child care personnel), and 5. The signature of a second child care personnel or person(s) authorized by the provider to verify the transportation log and that all children have arrived safely and left the vehicle (if applicable)."
Counselor was given a written notice from the provider that they will not be transporting children and haven't transported since June 2, 2023. They will resume transportation at an estimated date of 08/14/2023

33-03 · Technical assistance was provided to the director: Child Care Facility Handbook, Section 4.1 states, " Childcare personnel including volunteers who work 10 hours or more per month must begin training within 90 days of employment in the childcare industry and successfully complete Departments training within 12 months from the date training begins. Training taken prior to employment in the childcare industry does not constitute begin training. The begin training timeframes begin at the time of employment in the childcare industry. Training completion may not exceed 15 months from the date of employment in the childcare industry in any licensed Florida childcare facility. This may be accomplished by classroom attendance in a department-approved training course, acquiring an educational exemption from a department-approved training course, beginning a department-approved online childcare training course, or by receiving results from a department-approved competency examination. The childcare program is responsible for obtaining training documentation from childcare personnel."
S.M.Z. hired on 10/5/202; should have started the 40 Hours Introductory Training by 01/5/2023. This child care personnel began the training on 3/21/2023. As corrective action, S.M.Z. will provider written proof of beginning the 40 Hours Introductory Training

This was corrected as staff member listed on supplemental completed training that day. 8.2 Training Courses All program personnel, volunteers and substitutes must successfully complete one of the following sets of pre-service training coursework: 1. Completion of the department-approved online or in-person child care training courses listed below, as evidenced by successful completion of competency based examinations offered by the department or its designated representative with a weighted score of 70 or better. Information on training course access and availability can be found on the departments website at http://www.myflfamilies.com/service-programs/child-care/training. a. Each of the following: ? Health, Safety and Nutrition; ? Identifying and Reporting Child Abuse and Neglect; ? Child Growth and Development; and ? Behavioral Observation and Screening. 2. Completion of the below listed Early Learning Florida University of Florida (ELFL) health and safety modules, as evidenced by successful completion of competency based examinations offered by ELFL with a weighted score of 70 or better. Information on module access and availability can be found at https://www.earlylearningflorida.com/learningPlatform/user/login.lc. The below listed ELFL health and safety modules will be offered online and at no cost to providers and will be available no later than January 31, 2017. (Note: Providers must be in compliance with pre-service training requirements on or before March 31, 2017). a. Each of the following: ? Health and Sanitation; ? Safety of the Environment; ? Precautions in Transporting Children (if applicable); ? Safe Sleep Practices (not required for school-age only programs); ? Child Safety and Prevention; ? Planning for Emergencies; and ? Prevention of Child Abuse and Supporting Children in Trauma. b. One of the following developmentally appropriate practices modules: ? Supporting the Social-Emotional Development of Infants/Toddlers; ? Supporting the Social-Emotional Development of Preschool Children; or ? Supporting the Social-Emotional Development of Mixed-age Group Care. 3. Personnel employed by a public school district may show verification of completion of a course covering the identification and prevention of child abuse and neglect, which has been approved and administered by the school district, to meet the course requirement (s) above on the same subject matter

The counselor observed one employee [JC] had not started the introductory training within 90 days of employment. The employee's start date was 09/06/2022 and the training was not started until 01/30/2023. The standard was brought back into compliance because the training has been started. JC has until 12/06/2023 to complete the 40 hours introductory training and until 09/06/2023 to complete the early literacy training
