During the time of inspection, the licensing specialist observed ratio. Staff: Children 1:7 Mixed ages. 3 Infants, 1 Two-year-old and 3 Three-year-olds. Upon reviewing the child-to-staff ratio, the licensing specialist observed that the provider had exceeded the allowable number of children, placing the program out of compliance with required ratios. The licensing specialist informed the provider that she was out of ratio, provided technical assistance, and explained the importance of maintaining compliance with required ratios. The provider removed the mats from the rooms resolving the issue. During the time of inspection, the licensing specialist reviewed 1 child's file without an immunization record. The licensing specialist provided technical assistance and explained that vaccination records must be on file within 30 days of enrollment. FDCH/LFCCH Handbook, Section 8.1, A. The operator is responsible for obtaining for each child in care a current, complete and properly executed Florida Certification of Immunization form, Parts A-1, B, and/or C, DH 680 (July 2010), or the Religious Exemption from Immunization form, DH 681 (July 2008), which are incorporated herein by reference in 65C-22.001(7)(o) and (p), F.A.C., from the custodial parent or legal guardian, within 30 days of enrollment. DH Form 680 and DH Form 681 may be obtained from the local health department.
