The director advised that there are 4 drivers. Driver 1s license will expire on 6/1/29, the physical was conducted on 7/16/24, the CPR and First Aid certificate is valid until 2/28/27 and the driver has completed the transportation training. Driver 2s license will expire on 10/15/26, physical was conducted on 8/6/24, the CPR and First Aid certificate is valid until 11/30/25 and the driver as completed transportation safety training. Driver 3s license will expire on 12/23/29, physical was conducted on 3/19/2024, the CPR and First Aid certificate is valid until 11/30/25 and the driver as completed transportation safety training. Driver 4s license will expire on 9/16/26, physical was conducted on 1/31/24, the CPR and First Aid certificate is valid until 11/30/25 and the driver as completed transportation safety training. During the inspection, Licensing Specialist observed that 2 of the 4 drivers had an expired physical. The director was given until May 31, 2025, to come into compliance with this standard. 07-06 · Vehicle Requirements
During the inspection, the director advised that currently there are two transportation vehicles. First vehicle is a 2007 Chevy Midwest (133372) that was last inspected on 12/26/24 and is insured through Ace American Insurance Company with an effective date of 4/1/25. The second vehicle is a 2024 Chevy Bus (186782) that was last inspected on 4/15/25 and is insured through Ace American Insurance Company with an effective date of 4/1/25. 7-06 During the inspection, Licensing Specialist observed the 2007 Chevy Midwest (133372) that the installed vehicle alarm was not in working condition in that it could not be heard from the distance of 500 feet from the vehicle. The director was given until June 22, 2025, to come into compliance with this standard. During the inspection, Licensing Specialist provided technical assistance, regarding the maintaining of the vehicle alarm. CCF Handbook, Section 2.5.4, 1,D By January 1, 2022 all vehicles used by child care facilities to transport children must be equipped with a reliable alarm system approved by the Department which prompts the driver to inspect the vehicle for children before exiting the vehicle. 1. Care Provider Background Screening Clearinghouse: The Background Screening Clearinghouse will generate e-mails and send you, the provider, a reminder at 60, 30, and 21 days prior to the retention date expiration. Please note, you will only receive that notification for the staff that are on your facility/home roster. Please keep your roster current in the Clearinghouse to avoid confusion on expiration dates, or unwanted notifications. During the inspection, the Licensing Specialist reviewed the Background Screening Clearinghouse Rooster. During the inspection, Licensing Specialist observed one personnel did not have a cleared background screening that was working. The director put the personnel on leave until she had a cleared background screening which at the time of the inspection the director sent the personnel to have her fingerprints completed. During the inspection, Licensing Specialist provided technical assistance, CCF Handbook, Section 7.4.1, C Background Screening Documents Background screening documentation must be maintained for all child care personnel as defined by Section 402.302(3), F.S., which includes household members if the facility is located in or adjacent to the home of the operator. Background screening documentation must be on-site and available for the licensing authority to review. C. A copy of the eligible results, for the Level 2 screening, generated from the Clearinghouse must be on record for each personnel. If awaiting Out-of-State criminal history results, a copy of the DCF email informing of the individuals eligibility for a provisional hire status must be in the personnel file

Child Care Regulation observed two teachers with thirteen one- and two-year-old children. "(4) STAFF-TO-CHILDREN RATIO. (a) Minimum standards for the care of children in a licensed child care facility as established by rule of the department must include: 1. For children from birth through 1 year of age, there must be one child care personnel for every four children. 2. For children 1 year of age or older, but under 2 years of age, there must be one child care personnel for every six children. 3. For children 2 years of age or older, but under 3 years of age, there must be one child care personnel for every 11 children. 4. For children 3 years of age or older, but under 4 years of age, there must be one child care personnel for every 15 children. 5. 07-06 · Vehicle Requirements
Child Care Regulation observed that the alarms for both vehicles were not loud enough to be heard from a distance of 500 feet. 2.5.4 F. By January 1, 2022 all vehicles used by child care facilities to transport children must be equipped with a reliable alarm system approved by the Department which prompts the driver to inspect the vehicle for children before exiting the vehicle. 1. Approved alarm systems must meet the following criteria: a. The alarm system must be armed or activated automatically when the vehicles ignition is turned on. b. The alarm system must be designed and installed so that the vehicle horn, siren or other type of audio alarm will sound if the driver/staff member does not walk to the rear or, in the case of a passenger van, the side entry point of the vehicle, to manually shut off or deactivate the alarm. c. The time delay from the time the ignition is turned off after activation of the alarm system until the alarm sounds shall be no longer than one minute. d. The alarm must be audible from the distance of 500 feet from the vehicle. e. The alarm system must be installed so that the driver must walk to the back of the vehicle to reach the deactivation mechanism. Deactivation mechanisms installed in locations that do not require the driver to walk to the back of the vehicle and view all seating areas will not be acceptable. 2. List of approved alarm manufacturers may be found on the Departments website at www.myflfamilies.com/childcare . 3. Alarms must be installed and maintained according to the manufacturers recommendations. 4. Alarm systems installed according to the manufacturers instructions prior to October 1, 2021, will be deemed to satisfy this requirement if they meet the criteria set forth in 2.5.4.F.1. above. Under this allowance, the existing alarm system must always be properly maintained in working order. Should at any time the alarm system require replacement, the new system must be selected from the Departments approved list referenced in 2.5.4 F.2. above. The buses will have alarms that can be heard from a distance of 500 feet no later than June 22, 2023. Child Care Regulation will return after this date to confirm compliance

32-07 · Outdoor Equipment
Child Care Regulation observed that the ground covering was below the minimum requirement of six inches. During the inspection, Child Care Regulation observed that the facility did not maintain the mulch around the large climber, in that the mulch depth measured less than one inches in depth. 3.12, D. Permanent or stationary playground equipment must have a fall/use zone that extends a minimum of 6 feet in all directions from the perimeter of the equipment. All types of ground cover must be maintained to provide resilience and reduce the incidence of injuries to children in the event of falls. 1. If the ground cover in place is loose ground cover (such as, but not limited to: mulch, shredded rubber chips, or sand) a minimum of 6 inches in depth is required in the use zone. Asphalt, concrete, hard packed dirt, hay, grass or leaves are unsuitable for use in the use zone area. 2. If the ground cover in place is a unitary playground surface, then the unitary surfacing materials must be installed, maintained, or replaced according to manufacturers instructions. Unitary surfaces must be tested to and comply with ASTM F1292; documentation of test data must be retained at the facility and available for licensing to review. 3. If the play area was approved by the Department prior to January 1, 2020 and does not meet the 6-foot fall/use zone requirement, then the facility must submit a written notification, including photographs and layout of the play area, to the Department prior to or on April 1, 2020, notifying its intention to continue to operate using the prior approved play area. However, if the permanent or stationary playground equipment is moved or replaced then the standard would apply, and 6 feet of use zone must be provided. Any new equipment added would be required to have the 6 feet of use zone in all directions from the perimeter of the equipment. The facility will have six inches of a resilient surface no later than June 22, 2023. Child Care Regulation will return after this date to check for compliance
