34-05 · Credentialed Staff
License Specialist observed that The Credentialed Director was not present on-site a majority of the hours that the facility is in operation as documented on previous Inspection Reports. Technical Assistance was given to the Provider as per CCF handbook that ( Section 402.305(2)(g), F.S., requires a Child Care facility to have a credentialed director. Every applicant for a license to operate a Child Care facility or a license for a change of ownership of a child care facility must document that the facility director has an active Director Credential prior to issuance of the license. An applicant for the Director Credential must meet the requirements referenced in CFFSP Form 5290, Florida Child Care and Education Program Director Credential and Renewal Application, which is incorporated by reference in 65C-22.001(7)(k), F.A.C. CFFSP Form 5290 may be obtained from the Departments website at www.myflfamilies.com/childcare. All applications and documentation will be verified, and, if complete, the credential will be issued by the Department or its designated representative on CF-FSP Form 5252, Florida Child Care and Education Program Director Credential, which is incorporated by reference in 65C-22.001(7)(g), F.A.C. 04-01 · Supervision
Licensing Specialist observed as Child Care Personnel left the classroom several times to open the front door several times, at this point staff was not within sight or hearing of children in care. Technical Assistance was given to Provider as per the CCF Handbook that ( Child care personnel must be assigned to provide direct supervision to a specific group of children and be with that group of children at all times. Children must never be left without child care personnel supervision inside or outside the facility, in a vehicle, or at a field trip location. Supervision standards apply at all times away from the child care facility, including during field trips, outdoor play, and when picking up or dropping off children at designated locations, such as bus stops, school, or a childs home. For school-age children, personnel must know where the children are and what they are doing at all times and capable of responding to emergencies including when children are separated from their group. ) Violation was corrected at time of inspection when the owner arrived and Child Care Staff incorporated himself to the classroom and continued with scheduled activity for the specified time

Licensing Specialist observed upon arrival to Infant/1/2 Year Old classroom, Three (3) children were left in high chairs outside of feeding times. Technical Assistance was given to the Provider as per the CCF Handbook that ( Children must not be left in high chairs or other types of feeding chairs other than during feeding times. The use of safety straps is required to prevent children from falling out of the high chair. Violation was corrected at time of inspection by Child Care Personnel removing the children from the high chairs and beginning the morning activity as per the class schedule posted for the assigned time

12-02 · Facility Environment
Licensing Specialist observed several areas of the facility were in need of repair. 1. There were several water spots on ceiling tiles in Rooms: Kitchen Area, Lobby Exit Area, PreK 1-2 Classroom and common areas. 2. Broken and cracked floor tiles with jagged and sharp edges, extending from the Lobby, office and across to the common areas leading to the PreK 1-2 Classroom. 3. Peeling paint on the ceiling of the 4/5 Year Old classroom. 4. Baseboards all had peeling paint in the Infant/1 Year Old Classroom. Technical Assistance was given to the Provider as per the CCF Handbook that ( All child care facilities must be clean, in good repair, free from health and safety hazards and from evidence of, or presence of, vermin infestation. Indoor play areas must be inspected daily for basic health and safety and documented on a daily inspection log. Outdoor play areas must be inspected daily for basic health and safety. Any problems must be corrected before the play area is used by children. Documentation of the indoor play area inspection must be maintained for 12 months

17-02 · Technical assistance was given to the provider as per the CCF Handbook that (The outdoor play area must be clean and free from litter, nails, glass and other hazards.) NOTED: This shed is part of the Facility's Capacity Calculator as Classroom # 7, measuring 12' x 24' with a Room Capacity for 8 Children
License Specialist observed an outside shed located in the playground area. There is a small blue, wooden fence with a new gate that measures 3.8 ft. in height. The fence closes with a latch but did not have a lock. License Specialist opened the gate that led to the shed. Outside of the shed there were Four (4) rusted 12" inch rebars, Three (3) paint spray bottle cans, paint brushes, a hardened cement bag, Two (2) vacuum cleaners, and debris accessible to the children in care. License Specialist walked to the shed and the door was open. The indoor knob and strike plate was missing leaving the door to the inside of the shed open at all times unable to close shut. The inside of the shed was used as storage. There was excrement, evidence and the presence of rodent and vermin in the shed. There was also paint cans, ladders, tools and educational materials/furniture

32-03 · Outdoor Equipment
Licensing Specialist observed a red, iron, spring (car/boat) with peeling paint and rust exposed on the base of the equipment. Technical Assistance given to the Provider as per the CCF Handbook that ( All playground equipment must be securely anchored, unless portable or stationary by design, in good repair, maintained in safe and sanitary condition, and placed to ensure safe use by the children. Maintenance must include inspections conducted every month of all supports above and below the ground and of all connectors and moving parts. Documentation of maintenance inspections must be maintained for 12 months. All equipment used in the outdoor play area must be constructed and maintained according to manufacturers recommendations and allow for water drainage. Any open containers with water must be emptied immediately after use, i.e. pots, toys, or other equipment that collects water. )

FSC observed the facility did not have a current and approved annual fire safety inspection by the local fire authority. CCF Handbook, Section 3.8.2, A TA Unless statutorily exempted, all child care facilities must conform to state standards adopted by the State Fire Marshal, Chapter 69A- 36, F.A.C., Uniform Standards for Life Safety and Fire Prevention in Child Care Facilities. A copy of the current and approved annual fire inspection report completed by a certified fire inspector must be on file with the licensing authority.
