Child, E.E. (11/06/2021) had a chunk of hair missing from the back left side of his head. When the child was asked who done this, the child was repeating himself. When a woman came from the kitchen, the child pointed and said that was who had pulled his hair. FINDINGS: A complaint inspection was conducted on 9/10/2025 and a statement from the assistant director and teacher was received. Video footage was also reviewed. Per the assistant director, she has no issues or concerns with the teacher in question. Per the teacher, she only watched the group for a little while that day. She stated that she only watched the group from around 3:30PM until 5:00PM. There were no incidents with the child in question. VIDEO FOOTAGE OBSREVED: At around 3:25PM, teacher (YA) is seen pulling a child by the braid toward her and sitting him down on a chair. Per the video footage, at around 3:27PM, the child in question is seen climbing the napping cots and the teacher carried him down by grabbing him by the arm and placing her second hand on his head. Her back is turned to the camera so I can't confirm if she pulled his hair at that instance. This complaint is unsubstantiated as I cannot confirm if the missing patch of hair was due to the teacher's actions. A non-compliance is being issued because the teacher did not interact appropriately with another child that had braids

A complaint was received alleging that on 12/19./2022, a child, MS, was scratching the tables and got a few splinters stuck in his fingers (the child's middle finger and index finger) and clean up the child's fingers and put the bandages on the child's Fingers. The parent noticed a lot of blood coming out of the bandage, and decided to take the child the ACA Aventura Hospital, ER around 6:30PM, and when the child was seen by a doctor they stated that the child finger was impacted by something to cause layers of the skin, that shows the fatty white part of the skin, the Doctor performed a X-ray and no fractured bones. A complaint inspection was conducted on 1/4/2022 and a statement was received from the school Director CRA and assigned teacher SM. Per the assigned teacher, the incident occurred on the playground during after school hours when the child was rubbing his fingers on the wooden table on the playground. She heard him crying and went over to see what was wrong. She saw blood on the picnic table, a broken piece of wood between the child's nail, a cut his nail bed and on another finger. The teacher took the child to the restroom and she washed off the fingers, took the piece of wood out and the child's skin started to change color to a bluish/purple color and stopped bleeding after washing it off. The teacher placed a band aid over the child's finger and parent arrived shortly after to pick up the child. The teacher said this happened between 3:45 and 5:00PM. The accident/incident report and attendance record was checked for compliance. Per the incident report, the incident happened at 3:50PM and the child was picked up at 5:18PM. At least one hour and 20 minutes had passed by from the moment of occurrence and and parent pickup time. This complaint is substantiated as the parents were not immediately notified after the incident occurred. a notice of violation is being issued
