At time of inspection, Director called program owner to discuss reinspection. Owner stated that parts for slide repair were on back order at Home Depot. Licensing monitor requested verification that parts were ordered prior to non-compliance due date. Provider emailed a receipt for rubber mulch, not parts for repair. Licensing monitor arrived at facility to complete reinspection and conduct an observed fire drill. Per Director, drills are normally conducted utilizing a hand bell. Director states "I don't really know" the location of the fire panel, only the pull station. A complaint was received on 6/21/2024 alleging that No one has CPR, they are fake. A complaint inspection was conducted on 6/24/2024 and a statement from the Owner (DR) was received. Per the Owner, A previous staff member stole some staff files, including the staff First Aid/CPRs. On /6/25/2024, i received two copies of First Aid/CPR certificates for the staff that were present during my inspection on 6/24/2024, the owner and infant teacher. I contacted the instructor mentioned on the certificate to which he declared that those are not his certificates and the instructor's signature is not his signature. TA was provided that falsification of documents is unacceptable and may lead to further administrative action. This complaint is deemed substantiated
