At the time of the inspection visit the facility did not have a food acceptance log; per the teacher the catering company did not provide a log at the time the food was deliver. The facility failed to maintain a food acceptance log to verify that food was delivered at the correct temperature. TA, the food log must include the delivery date, time of arrival, quantity and types of food, verification by the recipient of adequate temperatures of food, and the name and signature of the recipient

Director self reported that Ms. SN without cause or reason, walked up to child, D. and slapped him once on the side of his face. After the incident, D. did not cry, and remained sitting in his chair. The Assistant teacher was present in the classroom and observed the incident. D. had no visible injuries. There is also video surveillance of the incident. Ms. SN was also observed via video footage aggressively pulling K. by the arm. K. passed by Ms. SN, at which time, she grabbed him by the arm and swung him aggressively in a circle without warrant. She then sat K. down in the chair. Also, during an incident, Ms. SN took her hand and began squeezing and shaking K's head. During which time, K's head was moving back and forth. Further video surveillance shows an incident involving child M. She was laying down for a nap. She had her legs up in the air while playing with her leg. Ms. SN slammed child, M's, leg down and began to shake her head. During another incident, Ms. Noble was putting a cot down and placing covers on the cot. She lifted child, MO, from the ground and threw him onto the cot. A site visit was conducted on 4/30/2024 and video footage reviewed with the Owner/Director. The teacher, SN, was observed doing all these things to the children via video surveillance. Per the Owner/Director, the teacher was terminated on Sunday, 4/28/2024 due to her behavior toward the children. Her last day of attendance was Friday, 4/26/2024. The Owner/Director self reported the incident and complied with her duty of reporting suspected abuse of a child. This complaint is Substantiated and a non-compliance is being issued as the employee failed to follow the written disciplinary policies
