At the time of inspection, licensing monitor became aware that provider is offering transportation for aftercare without prior authorization from the Department. Provider did not have documentation from the leasing company demonstrating proof of compliance with requirements. Licensing monitor waited for bus arrival to complete vehicle inspection. Leasing company being utilized by program is out of compliance with requirements as follows: -No proof of driver Clearinghouse background screening -No first aid/CPR training, per driver -No proof of current vehicle inspection -Additionally, the following vehicle/documentation conditions were: -Observed three seatbelts were broken (back three seats on left side,) -Two seats have holes -The childcare personnel accompanying the driver did not complete required transportation training, and -Transportation logs are incomplete, missing signatures at pickup and drop-off Provider has not been approved for transportation services.
