07-04 · Ratios of Personnel to Children Group: Sec. 7-33, Ordinance, DCF Handbook, and Sec

The school director self reported that two children were left unsupervised in their classroom for about 5 minutes while the teachers and the rest of the group went to the PE court. The teachers did a headcount while in the PE court and realized that the two children were not there. FINDINGS: Per the director (D.L), the parents were notified about the incident on the date of occurrence. The children were fine and had a normal rest of the day. Per the teacher's written statements received by the director, they had sent the children to use the restroom and were lining up to go outside. They thought the children were in the line but didn't notice they never came out. One of the teachers went on break and the other teacher led the children to the playground. VIDEO FOOTAGE: Video footage was reviewed and the following was observed; -The group was sitting in a circle on a carpet with 2 teachers. One child (C.W.)asked to go to the restroom and two more children (M.M. and I.L.)followed along. M.M. was seen joining the room again but C.W. and I.L. stayed behind in the restroom. The group lined up and headed out to the playground at around 10:34 and by 10:35AM the group had left the room, Per the video time stamp. The children came out of the restroom a few seconds later to find themselves alone. The children were found at approx. 10:56AM. This complaint is substantiated and a class 1 non-compliance and Notice of Violation is being issued
