During the course of a complaint inspection related to transportation, the following was observed; The van was observed pulling up to the school, dropping children off and leaving again. Some minutes later, the van came back with one additional child and the driver. No other staff member was in the vehicle. After speaking with the director, she stated that the driver is the son of a teacher that works at the school. He did the school a favor to transport the children but did not have a file and no proof of background screening. A class 1 non-compliance and Notice of Violation is being issued

During the course of a complaint inspection regarding transportation, the following non-compliance was observed; The van was observed doing a drop off at 3:25PM. The driver unloaded the child and knocked on the entrance door. A teacher (V. D.) opened the door, took the child inside and closed the door. I asked the director for the transportation log for all the children that were transported into the facility. She responded that the owner was at the school earlier and he took the log with him. So I stated that i just saw one child get dropped off at 3:25PM and if she had a log for her to which she said no. I asked her to send me the transportation log she said the owner took with him. A transportation log was received that documented all the children who were transported, including the child that was transported at 3:25PM. The log had a signature on the driver and second person signature spaces. I had observed this child being dropped off and the driver and second person did not have a log nor conduct a sweep. A complaint allegation was received alleging that the school was suspected if providing transportation services without prior approval. FINDINGS: - The facility was licensed on 8/6/2025, and was not approved for transportation. The school was observed transporting children of ages 3 to 8 during today's visit. The vehicle being used has not been inspected and approved by child care licensing and fails to meet minimum licensing standards. There are transportation authorization forms on file for the children being transported. Provider was instructed to cease transportation services until it is approved by CCLE
