Complaint alleges that a child aged 1 year and 4 months was seated at an adult chair, by the teacher assistant, BG. The child fell off the chair and injury was a swollen lip and the child had bruises on right side. The area where the children were located, was not suited to have children. FINDINGS: A complaint inspection was conducted on 10/28/2025 and statements were received from the director and teacher. Per the director, The incident occurred during chapel. there is a building adjacent to this building and that is where they do chapel and where the incident happened. Chapel occurs from 9:00AM to 9:30AM. It is believed that the incident occurred around 9:15AM. Per the teacher, she put the child on the chair and the child slipped out of it and unto the floor. She had the child on her lap and sat him down on the chair to assist two other children that were fighting. Although she tried to hold the child back with her leg in front of the chair, the child slipped down to the floor. This complaint is substantiated. The chairs that were being used to sit the children are not age appropriate and the temple being used for chapel is not an approved space. TA was provided to cease use of this space and to only used spaces that have been approved by Child Care Licensing and Enforcement
