During the inspection, Licensing Specialist observed on the playground the meatal gate by the blue building was open and does not close and it leads to the outside gate where it was open and a black truck was on jacks which is accessible to the children in care and on the other side of that gate there is buckets, trash cans, black plastic items and broom handles laying on the ground(the provider fixed the fence to where it could shut and lock). The wooden gate that is by the blue building is unlocked and the children have access to the following items: oil, 7 5 gallon driveway filler sealer, spray paint, wires and it also leads to the outside(the provider locked the fence). There is a red electrical wire(that has been cut) sticking out of the ground by the bench that is next to the white shed(the provider buried the red wire at the time of the inspection). The electrical box that is connected to the electrical pole by the white shed is broken and sticking out exposing the wires inside and there is a dipping sauce inside it(the provider replaced the electrical box). This is also accessible to the children in care. There 2 benches by the white shed that the wooden panel are lifting up off of it(The provider removed one bench and repaired the other bench). There are wooden fence panels laying on the ground next to the fence that has nails sticking out(The provider removed the panels from the playground and threw them away). At the bottom of the blue building under the las set of windows by the white door there is a spot of the wood that is deteriorating. The left side of the door panel of the white door on the blue building is deteriorating at the bottom and the the door henge is rusting. There is a wooden fence panel that is broken(The provider placed a new panel onto the fence). On the metal fence in the back there is a metal wire sticking out the top of it(The Provider removed the metal wires). On the wooden jungle gym there is a board at the top in the middle section that has a spot deteriorating and exposing the screws. There is a green substance on the white shed and the white door that is connected to the blue building. The air condition that is connected to the blue building is rusted at the bottom. The metal cover over the window that is closet to the white door on the blue building is loose. The provider was given until June 8, 2026, to come into compliance with this standard. During the inspection, Licensing Specialist observed that the last fire drill was not conducted during facility hours. The fire drill was conducted during school hours not during Web of Wisdom hours of operation. The last fire drill was conducted on 5/15/24 at 2:54 pm. During the inspection the provider could not bring this back into compliance due to not knowing the code to the alarm. During the facilitys license year, fire drills utilizing the alarm system, approved by the local fire authority, must be conducted monthly at various dates and times when children are in care. B. When the facilitys approved fire alarm alert system is activated or initiated, all adults and children must evacuate the facility. C. A current attendance record and parent contact information must accompany child care personnel out of the building during a drill or actual evacuation and be used to account for all children. The operator must maintain a written record of the fire drills showing the date, number of children and child care personnel in attendance, evacuation route used, and time taken for all individuals to evacuate the premises. Each fire drill record must be maintained for a minimum of 12 months from the date of the fire drill. The fire drills conducted must include, at a minimum: 1.
