33-06 · Enrollment Information/Daily Attendance FDCH/LFCCH Handbook
During the time of the inspection, the Owner/Operator/Director advised the Licensing Specialist that there are currently 4 children enrolled. During the time of the inspection, the Licensing Specialist monitored 4 of the 4 childrens records. Licensing Specialist monitored the September 2024 Distracted Driver Brochures. Licensing Specialist monitored the August/September 2024 Influenza Brochures. 33-6: During the inspection, Licensing Specialist observed that the daily attendance was not accurate for 1/9/2025 in that there were 4 children on site and zero children were signed in. During the inspection, the provider signed the 4 children. This is being marked corrected at the time of the inspection. During the inspection, Licensing Specialist provided technical assistance, FDCH/LFCCH Handbook, Section 8.4, A and B A. Daily attendance of children must be taken and recorded by child care personnel, documenting the time when each child enters and departs the home. B. During the inspection, Licensing Specialist observed 1 childs record did not have a current immunization on file. During the inspection, Licensing Specialist provided technical assistance, The operator is responsible for obtaining for each child in care a current, complete and properly executed Florida Certification of Immunization form, Parts A-1, B, and/or C, DH 680 (July 2010), or the Religious Exemption from Immunization form, DH 681 (July 2008), which are incorporated herein by reference in 65C-22.001(7)(o) and (p), F.A.C., from the custodial parent or legal guardian, within 30 days of enrollment. DH Form 680 and DH Form 681 may be obtained from the local health department. The parent/guardian of a child who has not received the age-appropriate immunizations prior to enrollment and who does not have documented medical or religious exemptions from routine childhood immunizations should provide documentation of a scheduled appointment or arrangement to receive immunizations. 27-4: During the inspection, the fire drill log was not maintained in that the October 2023 fire drill was not documented on the fire drill log. During the inspection, the provider updated the log showing that a fire drill was conducted on 10/17/2023. The is being marked corrected at the time of the inspection. During the inspection, Licensing Specialist provided technical assistance, A current attendance record and parent/guardian contact information must accompany child care personnel out of the home during a drill or actual evacuation, and be used to account for all children. The operator must maintain a written record of the fire drills showing the date, number of children and child care personnel in attendance, evacuation route used, and time taken for all individuals to evacuate the premises. Each fire drill record must be maintained for a minimum of 12 months from the date of the fire drill. The fire drills conducted must include, at a minimum: 1. 6-1: During the inspection, Child Care Regulation observed the Background Screening for the owner and the 2 household members and saw that their Background Screening is expired. The owner advised that they will be on vacation from June 7, 2023 through June 27, 2023, so additional time is being provided to come back into compliance. 8-1: During the inspection, Child Care Regulation observed that the 6 children in care were being supervised by an unscreened individual, the owner. During the inspection, Child Care Regulation had the owner call the custodial parents to come pick up their children. Child Care Regulation advised that the owner cannot watch any children until she gets an eligible result for DCF child care on the Background Screening Clearinghouse or the owner can have her substitute come in a watch the children at her home. 1:9 (1 infant, 4 one year olds, 1 two year old and 3 three year olds) 1-01: Child Care Regulation asked the owner how many children she had in attendance and the owner advised that she had 6 children. Child Care Regulation counted 6 children (5 in the living room and 1 in the back room). Child Care Regulation walked through out the interior of the home, checking all the rooms and closets. The owner provided 6 files. Child Care Regulation walked outside to the outdoor play area in the backyard. Child Care Regulation walked around the corner of the house and observed 2 household members standing at the end of the fence with 3 more children. Child Care Regulation observed that the 3 children were without Child Care Personnel supervision outside of the home in the backyard. Child Care Regulation observed a total of 9 children at the home (1 infant, 4 one year olds, 1 two year old and 3 three year olds). During the inspection, Child Care Regulation told the owner that 3 children have to be sent home to get back into compliance regarding ratio. The owner contacted three authorized pick-up people to come and pick up the children. During the inspection, three children went home. This is being marked corrected at the time of the inspection. During the inspection, Child Care Regulation provided technical assistance, (8) Family day care home means an occupied residence in which child care is regularly provided for children from at least two unrelated families and which receives a payment, fee, or grant for any of the children receiving care, whether or not operated for profit. Household children under 13 years of age, when on the premises of the family day care home or on a field trip with children enrolled in child care, shall be included in the overall capacity of the licensed home. A family day care home shall be allowed to provide care for one of the following groups of children, which shall include household children under 13 years of age: (a) A maximum of four children from birth to 12 months of age. (b) A maximum of three children from birth to 12 months of age, and other children, for a maximum total of six children. (c) A maximum of six preschool children if all are older than 12 months of age. (d) A maximum of 10 children if no more than 5 are preschool age and, of those 5, no more than 2 are under 12 months of age. The Child Care Regulation Supervisor advised that the household member tried the leave through the garage a few more times but could not leave due to her standing there. The owner advised that she was trying to hind the three children because she new that she had to many children. This is being marked corrected at the time of the inspection. During the inspection, Child Care Regulation provided technical assistance, The operator shall remain responsible for the supervision of the children in care and capable of responding to emergencies and the needs of the children at all times. Child care personnel must directly supervise children, both indoors and outdoors, by sight and sound. Children must never be left without child care personnel supervision inside or outside the home, in a vehicle, or at a field trip location by themselves
