4-15: During the inspection, Licensing Specialist observed the there was a three-year-old in the bathroom that needed assistance and there was not an employee available to assist the child. The child was unable to find any toilet paper in the bathroom stall. Licensing Specialist assisted the child. The bathroom is located at the other end of the hall from the three-year-old classroom. The three-year-old went back to is classroom after washing his hands. This is being marked corrected at the time of the inspection. 12-02 · Facility Environment
12-2: During the inspection, Licensing Specialist observed that there were areas in the facility that were not in good repair. In the two-year- old classroom the walls were peeling. The accordion door was off its track in the hallway. In the VPK 1 classroom, there was small holes in the wall, walls peeling, and the sinks frame was peeling. In the VPK 2 classroom, a portion of the wall was peeling. In the three-year-old classroom, one of the slates on the closet door was coming off. There was also tape on the walls and stalls in the bathroom. During the inspection, Licensing Specialist provided technical assistance, CCF Handbook, Section 3.1, A All child care facilities must be clean, in good repair, free from health and safety hazards and from evidence of, or presence of, vermin infestation. Indoor play areas must be inspected daily for basic health and safety and documented on a daily inspection log. Outdoor play areas must be inspected daily for basic health and safety. Any problems must be corrected before the play area is used by children. Documentation of the indoor play area inspection must be maintained for 12 months

The prescription was filled on 11/25/2024 and the direction state to apply the prescription for 7- 10 days. 6-01: The director advised that there are 3 drivers. Driver 1s license will expire on 2/14/2028, physical was conducted on 7/24/2024, the CPR and First Aid certificate is valid until 8/31/2025 and the driver 1 has completed the transportation training. Driver 2s license will expire on 2/24/2028, physical was conducted on 8/19/2024, the CPR and First Aid certificate is valid until 8/31/2025 and the driver as completed transportation safety training. Driver 3s license will expire on 8/21/2030, physical was conducted on 8/22/2024, and the driver as completed transportation safety training. During the inspection, Licensing Specialist observed that Driver 3 did not have a current CPR/First Aid card/certificate. 34:51 Teacher #1 is mostly out of the classroom, then completely out of the classroom and in the hallway. She is unable to see the children. At 2:39:11, the teacher that gave Teacher #1 a lunch break left the room when Teacher #1 came back. At 2:40:02, Teacher #1 left the room with a child and closed the classroom door. She came back in the room at 2:45:54, Teacher #1 left the room again at 2:46:03 and closed the door partially. At 2:49:13 an adult walked into the room and stood in the doorway. At Teacher #1 came back in room at 2:51:53. Teacher #1 left room at 3:25:53 and come back at 3:28:43. Teacher #1 left the classroom at 3:32:45 and came back at 3:33:40. The following ratios were observed during the walk through: 2:20 VPK. 2:12 Three-Year-Old. 2:6 One-Year-Old, Nap Time. 0:10 Two-Year-Old, Nap Time. The following ratio was observed in the Two-Year-Old classroom 1:10 when the teacher returned to her classroom thus returning this standard into compliance at the time of this inspection. During Nap Time Licensing specialist observed 2 childcare personnel in the One-Year-old class. AB, was observed knelling down calming down a child that was crying in the One-Year-Old class. AB, left the One-Year-Old classroom and returned to the Two-year-old classroom and was in charge of 10 children by herself. Licensing specialist asked, AB, if she had left these children alone when she was helping in the One-Year-Old class, and she said; "yes." The class was out of ratio due to the teacher being physically in the One-Year-Old classroom. FL Statutes Section 402.305(4), F.S.(4) STAFF-TO-CHILDREN RATIO: (a) Minimum standards for the care of children in a licensed child care facility as established by rule of the department must include: 1. For children from birth through 1 year of age, there must be one child care personnel for every four children. 2. For children 1 year of age or older, but under 2 years of age, there must be one child care personnel for every six children. 3. For children 2 years of age or older, but under 3 years of age, there must be one child care personnel for every 11 children. 4. For children 3 years of age or older, but under 4 years of age, there must be one child care personnel for every 15 children. 5. For children 4 years of age or older, but under 5 years of age, there must be one child care personnel for every 20 children. 6. For children 5 years of age or older, there must be one child care personnel for every 25 children. 7. During Nap Time Licensing specialist observed 2 childcare personnel in the One-Year-old class. AB, was observed knelling down calming down a child that was crying in the One-Year-Old class. AB, left the One-Year-Old classroom and returned to the Two-year-old classroom and was in charge of 10 children by herself. Licensing specialist asked, AB, if she had left these children alone when she was helping in the One-Year-Old class, and she said; "yes." Teacher and Owner informed Licensing Specialist that they misunderstood Nap Time Supervision. TA was provided: Licensing Specialist explained Nap Time Supervision and requirements. All child care personnel are to be within sight and hearing of all the children during nap time. Teacher was back in the Two-Year-Old Classroom, within sight and hearing of all children in that class , thus returning this standard into compliance at the time of this inspection. CCF Handbook, Section 2.4.2 Nap time Supervision: During nap time,: child care personnel must be within sight and hearing of all the children. All other child care personnel required to meet the staff-to-child ratio must be within the building on the same floor and available to be summoned if needed to ensure the safety of the children. Licensing Specialist observed the employee that was hired on 9/6/2022 did not start or finish the Department approved five-hour early literacy and language development course for child care personnel within 12 months of date of employment in child care industry. TA was provided: employee registered for ELC course at the time of this inspection. 4.2.2 Early Literacy Training Pursuant to Section 402.305(2)(e)5., F.S., all child care personnel must complete a single course of training in early literacy and language development of children ages birth through five years that is a minimum of five clock hours or .5 CEUs.
