Upon LS arrival, the ceiling in the cafeteria was observed to no be in good repair. Ceiling tiles were missing and there was a leak from the ceiling. All child care facilities must be clean, in good repair, free from health and safety hazards and from evidence of, or presence of, vermin infestation.
