TA: Each room routinely used as a classroom must provide the minimum 35 square footage of usable indoor floor space per child. The 2 years old classroom have a capacity on 12 children and 14 children were observed in the classroom at time of inspection. Provider has resolved the non-compliance at the time of the inspection by removing two children and placed them in the 4 years old

39-04 · Accident/ Incident Notification and Documentation

The child is no longer at the facility. Incident report was also written and placed in the other child's folder. As technical assistance, this documentation must be shared with the custodial parent or legal guardian on the date of occurrence. The documentation must be maintained for 12 months. If the parent or legal guardian does not pick up the child on the date of occurrence of the accident or incident, the individual authorized to pick up the child must sign and be provided a copy of the accident/incident form
