While upstairs after soccer shots the two year, three year, and four year old classes were combined with one teacher upstairs. There were 11 two year olds, 15 three year olds, and 20 four year olds. The majority of the children were four, therefore, a ratio of 1:20 was required. To correct the ratio problem a second teacher came upstairs and took the two year old children downstairs in the elevator. A third teacher came upstairs to assist with ratio and to get the children back downstairs to their classrooms. The corrected ratios were: 1:11 Twos 2:35 Threes Fours (majority fours)

47-01 · Access/Child Safety

Per interviews with child care personnel and video recording verified inappropriate interactions and demeaning words were observed towards the child. Per interviews with staff the child was not following instructions, however, the child was not a harm to himself or those around him. Two child care personnel dragged the child by the arms along the floor and placed the child in the elevator to go down stairs. The Provider will complete an Appropriate Interaction with Children training with all childcare personnel. The Provider will submit training itinerary and sign in roster to the licensing office on or before 06/26/2023 via email

33-02 · Training Requirements

The employees listed on the supplemental form did not have their DCF transcript printed and placed in the employee file. The Provider will print the employees DCF transcript and submit copies to the licensing office via email, and place them in the employee's files. Submit DCF transcripts on or before 03/03/2023 to verify compliance. The employee will complete the required five hour early literacy course on or before 03/03/2023.
