Complainant alleges that MC stated that she had fallen off the swing onto the floor. She indicated she hurt her back and was crying. According to MC, the "teacher" Ms. AM told her "don't sit on the floor all day" and not to cry. MC said she stopped crying despite it hurting because she didn't want to go to "time out". The parent was not informed of the fall at any point. Statements from the staff (director and teachers) were received. Per director, she was not here on the date of the incident. She stated that she spoke to the teacher and the teacher told her that the child fell from the swing sand she called her over and soothed her. Per the afternoon teacher(AM), the incident happened on Tuesday or Wednesday of last week. She was on the swing. She was falling off so another teacher was calling her and she didn't come. So she called her and asked her what happened so MC told her she fell. She asked her if she needed an ice pack or anything and the child said no and went back to play. She stated that Ms. VM was calling her to come after she fell but she refused to come. She was not the teacher at the time. She was there because her group was also out there on the playground. Per the child's teacher (VM), She stated that the child was sitting on the swing and she slid to the floor so she told her to get up and not sit on the floor like that. The child just looked at her and she walked away because the child didn't want to be bothered. She was not crying, sad nor mad. An incident report was not created because she asked her if she was alright and the child didn't respond. Complaint is substantiated and a non-compliance is being issued for not documenting the incident and giving it to the parent on the date of occurrence
