A complaint allegation was received on 5/18/2023 alleging that on 5 /17/23 after parent picked up child, she unzipped the jack and saw blood stains on the shirt and laces on the seat pants. Parent noticed the child had a busted bottom lip inside of child's mouth. The parent was still in parking lot at provider parent wiped off blood herself went back in the school to ask about the scratches, parent says she was speaking with lady who does the registration she believes the director , the director calls the teacher to ask about the scratches the teacher says block was broken and it was not put on incident report. A complaint inspection was conducted on 5/19/2023 and a statement from the teachers(EO and BG) and director (SB). Based on the statement received from the teacher(EO), She is unaware of an incident at the school that could have caused the busted lip on the child. She stated that the cut didn't look fresh to her. She didn't see any stains on the child's shirt and he had a jacket on for most part of the day. The scratches on the child's cheek came form a green block the child was playing with on 5/11/2023 and they were already healed. The child scratched himself because it was broken. Based on the statement received from the teacher(BG), she only covered the teacher's lunchbreak on 5/17/23. Nothing happened during naptime as the children were napping. Per Director and owner, the cameras do not record. This complaint regarding the busted lip could not be substantiated. The complaint regarding the scratches on the face is substantiated. A non-compliance is being issued for the scratches that occurred on 5/11/2023 because there is no proof of an accident/incident report from the date of the incident
