27-01 · Fire Drills/Emergency Preparedness FDCH/LFCCH Handbook

There was no documentation showing a fire drill was conducted in December 2023 and January 2024. The Provider will complete two make up drills in addition to conducting the required February 2024 fire drill. The Provider will submit the current fire drill log to the licensing office on or before 03/12/2024 via email to verify compliance

07-10 · Staff Training FDCH/LFCCH Handbook

The Provider's CPR and first aid expired 07/14/2023. The Provider will complete a CPR and first aid course on or before 08/28/2023 and submit to the licensing office via email to verify compliance. Per DCF Handbook: C. Have certificate(s) of course completion for pediatric cardiopulmonary resuscitation (CPR) procedures and first aid training, which must be current and valid at all times. Certificates of course completion are valid based on the time frames established by each first aid and CPR training program, not to exceed three years. CPR courses must include an on-site instructor-based skills assessment that shall be documented by the certified CPR instructor. Documentation of completion of the online course and on-site assessment must be maintained at the home and available for review by the licensing authority

32-01 · Health Records FDCH/LFCCH Handbook

The children listed on the supplemental form need student health forms. The Provider will obtain current student health records from the parent guardian on or before 03/01/2023. Submit current student health records to the licensing office via email by 03/02/2023
