DCF Handbook, Section 3.1, A All child care facilities must be clean, in good repair, free from health and safety hazards and from evidence of, or presence of, vermin infestation. Indoor play areas must be inspected daily for basic health and safety and documented on a daily inspection log. Outdoor play areas must be inspected daily for basic health and safety. Any problems must be corrected before the play area is used by children. Documentation of the indoor play area inspection must be maintained for 12 months. The ceiling tiles in several rooms are stained, and the ceiling lights need to be free of dust and insects. Per DCF Handbook A log must be maintained for all children being transported in a vehicle or on foot away from and/or to the premises of the child care facility. The log must be retained on file at the facility for a minimum of 12 months and available for review by the licensing authority. The log must include: 1. Each childs name, 2. The date and time of departure, 3. Time of arrival at the destination, 4. The signature of the driver (or in the case of traveling on foot, the signature of the childcare personnel), and 5. The signature of a second child care personnel or person(s) authorized by the provider to verify the transportation log and that all children have arrived safely and left the vehicle (if applicable). Prescription medication must have a label stating the name and contact information of the physician and/or pharmacy, childs name, name of the medication, and medication directions. All prescription and non-prescription medication must be dispensed according to written directions on the prescription label or printed manufacturers label and maintained at the appropriate temperature. 04-01 · Supervision
On 4/3/23, [Person 1] was removed from [Child 1's] pick up list. Child's mother states she emailed the daycare to remove (Person 1) from any contact with [Child 1]. On 4/19/23, [Staff A] allowed [Person 1] to take [Child 1] into the bathroom of the classroom and change the child's clothes. [Staff A] stated that she was unaware that [Person 1] was no longer on the pick up list and that the mother did not want her to have contact with the child. Management states they put sticky notes in teachers' mailboxes regarding the change and [Staff B] states she spoke to staff face to face regarding the change. [Child 1] was transferred to another classroom and all teachers were notified again that there is to be no contact or communication between [Person 1] and [Child 1]
