The carpet transition strip in the three-four year old classroom has came up and hanging over onto the tiled floor. The Provider will have the carpet transition strip repaired on or before 03/28/2023. Submit pictures of the repaired flooring to the licensing office via email to verify compliance

Seven bottles stored in the refrigerator were not labeled with the child's first and last name. The Provider labeled the bottles at the time of inspection. Per DCF Handbook: B. Bottles and sippy cups brought from home shall be individually labeled with the childs first and last name and shall be returned to the custodial parent or legal guardian daily

32-07 · Outdoor Equipment
The Provider needs to add resilient surface under the green spinning toy to meet fall zone requirements. The Provider will send pictures to the licensing office to verify additional resilient surface has been added to meet fall zone requirements by 03/28/2023 via email. Per DCF Handbook: D. Permanent or stationary playground equipment must have a fall/use zone that extends a minimum of 6 feet in all directions from the perimeter of the equipment. All types of ground cover must be maintained to provide resilience and reduce the incidence of injuries to children in the event of falls. 1. If the ground cover in place is loose ground cover (such as, but not limited to: mulch, shredded rubber chips, or sand) a minimum of 6 inches in depth is required in the use zone. Asphalt, concrete, hard packed dirt, hay, grass or leaves are unsuitable for use in the use zone area
