The Licensing Specialist Reviewed 5 records including household members. At time of the inspection, the Licensing Specialist observed, two members did not have a current C. CF-FSP Form 5337, Child Abuse & Neglect Reporting Requirements, that was signed annually. The forms had been signed by the staff more than a year before the date of inspection. The Licensing Specialist took pictures. The Licensing Specialist provided technical assistance from the handbook Section 4.3,H. The operator, substitute, employee and/or volunteer must complete a CF-FSP 5337, Child Abuse and Neglect Reporting Requirements, incorporated by reference in 65C-22.001(7)(l), F.A.C., signed prior to initial licensure and annually thereafter
