At the time of the inspection the counselor observed one staff records did not have documentation of five years of employment history in the file. Provided technical assistance from the handbook 5.1, D The employer/owner/operator must conduct employment history checks, including documented attempts to contact each employer that employed the individual within the preceding five years, and documentation of the findings. Documentation must include the applicants job title and description of his/her regular duties, confirmation of employment dates, and level of job performance. The employer/owner/operator must make at least three attempts to obtain employment history information. Failed attempts to obtain employment history must be documented in the personnel file and include date, time, and the reason the information was not obtained

At the time of the inspection the counselor observed a hammer in the drawer in the room where children had been eating lunch. The drawer did not have a child lock and was a under a countertop in the room where a child could have access. Pictures taken. The counselor showed the director who immediately moved the hammer to a high place above a cabinet. resolving the issue. The director explained the high cabinet where the hammer is normally stored came down over the weekend. Provided technical assistance from the handbook section 3.2.B B. All potentially harmful items, including cleaning supplies, flammable products, poisonous, toxic, and hazardous materials, must be labeled and used according to manufacturers recommendation. These items, as well as knives, sharp tools, and other potentially dangerous hazards, must be stored in a locked area or must be inaccessible and out of a childs reach at all times
