04-18 · Supervision
During the inspection, the Licensing Specialist (LS) learned five children in the age one and two classroom were being supervised by a new staff member who did not have cleared background screening. The staff member was waiting for the results of fingerprinting. The director resolved the issue by promptly sending the staff member home and taking over supervision the children in that classroom. The (LS) explained to the director that staff must meet all the requirements in statue s. 435.06(2)(a) prior to any unsupervised contact with children. The staff member must have eligible results for fingerprints not still in process. Additionally explained that if the staff member had a provisional letter from background screening, they would still need to be supervised in the classroom until final clearance. Provided technical assistance from the handbook Section 7.4.1 Background Screening Documents Background screening documentation must be maintained for all child care personnel as defined by Section 402.302(3), F.S., which includes household members if the facility is located in or adjacent to the home of the operator. Background screening documentation must be on-site and available for the licensing authority to review. A. Each personnel record must have a completed CF-FSP Form 5131, Background Screening and Personnel File Requirements, which is incorporated by reference in paragraph 65C-22.001(7)(b), F.A.C. B. CF Form 1649A, Child Care Attestation of Good Moral Character, which is incorporated by reference in 65C-22.001(7)(a), F.A.C., must be completed for all child care personnel at the time of initial screening or upon change in employers. CF Form 1649A may be obtained from the Departments website at www.myflfamilies.com/childcare. C. A copy of the eligible results, for the Level 2 screening, generated from the Clearinghouse must be on record for each personnel. If awaiting Out-of-State criminal history results, a copy of the DCF email informing of the individuals eligibility for a provisional hire status must be in the personnel file. D. A copy of the DCF letter/email informing of search conducted of the Florida child abuse and neglect registry must be on record for each personnel screened between July 1st and December 15th of 2016. E. A copy of each request made to out of state child abuse and neglect registries for individuals who lived outside the state of Florida in the preceding five years. F. A copy of each search conducted for out of state sexual offender/predator registries for individuals who lived outside the state of Florida in the preceding five years

19-02 · Nap/Sleep Space Requirements
During the inspection, the Licensing Specialist (LS) observed children in all the classrooms that were napping with less than an18 inch separation. The (LS) took pictures. The Director had the staff move the napping children's cots further apart and the center and resolved the non compliance. The Licensing Specialist provided technical assistance from the handbook section 3.6.2, C.A minimum of 18 inches must be maintained around individual napping and sleeping spaces. A maximum of two sides of a napping or sleeping space may be against a solid barrier, such as the wall. The solid side of a crib does not meet the requirements for a solid barrier

30-07 · Diapering
At the time of the inspection, the Licensing Specialist(LS) observed a small green bin with plastic toys on the diapering station in the room with the age one and two children. The (LS) took pictures. The director educated the staff and the green bin was removed bringing the center into compliance. The Licensing Specialist provided technical assistance from the handbook section 3.10.2.G G. Items unrelated to diaper changing shall not be stored in the diaper changing area nor shall they be placed on the diaper changing table

45-02 · Background Screening Documents
At the time of the inspection, the Licensing Specialist (LS)observed one staff member did not have documentation of Level 2 Clearinghouse screening in the file. The staff member fingerprints were in process. The provider stated they thought the staff was approved provisionally. The (LS) explained the provider would need to have the provisional email in the record to document the department granted a provisional hire status. Prior to the completion of this report the provider emailed the licensing specialist documentation showing the staff member was eligible as of 12/18/2024. The licensing specialist reviewed the results on the background screening site that showed the staff member now eligible as of 12/18/2024. At the start of the inspection the licensing specialist observed 2 staff supervising 16 children Age 1 to 3 at nap time. The specialist advised the director that there are no exceptions for ratio with any children under age 2. For children 2 years of age or older, but under 3 years of age, there must be one child care personnel for every 11 children. 4. For children 3 years of age or older, but under 4 years of age, there must be one child care personnel for every 15 children. 5. For children 4 years of age or older, but under 5 years of age, there must be one child care personnel for every 20 children. 6. For children 5 years of age or older, there must be one child care personnel for every 25 children. 7. At the time of the inspection, the counselor observed two child's file had an expired immunization record. Provided technical assistance from the handbook Section 7.1, Immunization Records The child care facility is responsible for obtaining for each child in care a current, complete and properly executed Florida Certification of Immunization form Part A-1, B, or C, DH 680, which is incorporated by reference in 65C-22.001(7)(o), F.A.C., or the Religious Exemption from Immunization form, DH 681, which is incorporated by reference in 65C22.001(7)(p), F.A.C., from the custodial parent or legal guardian. DH Form 680 and DH Form 681 may be obtained from the local county health department. Specific immunization requirements are included and detailed in the most current edition of the Immunization Guidelines-Florida Schools, Child Care Facilities and Family Day Care Homes as promulgated by the Florida Department of Health.
