During today's inspection the following was observed in need of repair: -The wooden gate leading to the front yard was observed open and without a lock or ability to close as the latch was broken. -The wooden gate leading to the pool was observed open and the locking mechanism was inoperable. -A pole on the fence that encloses the pool was observed leaning inward and unattached to the ground. -The wooden fence had holes, sharp edges, and missing screws near the base of the fence throughout the playground. -The outside table had holes and jagged edges on all corners. -The playground area had debris and trash throughout the play area. -Exposed pipes on playground with standing water needs to have a barrier and water cleared out

The operator did not have a record of the vehicle's inspection/vehicle insurance and registration, and the vehicle fire extinguisher was tagged March 2023. She stated that she is not currently transporting children. At the time of arrival, the operator was gathering the children near the door and she stated that they were getting ready to go to the park. We inquired about how the children were to be transported to the park to which she responded that a friend/neighbor would take them because her van is not working at the moment. The operator was reminded that all vehicles used to transport children must be approved for transportation and meet minimum standard requirements. She was advised that the van will be inspected at the revisit inspection on 10/31/2024 to ensure all transportation standards are in compliance
