A complaint allegation was received alleging that a previous complaint allegation was submitted to CCLE on 5/21/2024. Complainant reported that she witnessed the children being left unattended. Children are consistently coming home with bruises due to other children hitting them, and other children hitting and kicking each other with no adult interference. This complaint allegation was previously investigated on 5/22/2024 and investigation has been closed. A follow-up revisit was conducted today to verify compliance for the previous non-compliances issued on 5/22/2024. On today's visit, the provider is in compliance with supervision but out of compliance for ratio. The mixed group of 1y/o and 2y/o was out of ratio in that one of the two teachers stepped out of her room to open the main entrance. TA was provided that the person that opens the door cannot be a person that is required to be in the classroom for ratio purposes. This is the second occurrence since the last inspection, 5/22/2024. Therefore, A notice of violation is being issued

A complaint allegation was received alleging that children are being left unsupervised. A complaint inspection was conducted and after speaking with a teacher Ms. EP, I determined that the teacher was in charge of supervision the 4s classroom. The teacher walked out of the classroom to tend to the door upon my arrival. I asked the teacher if she was needed in the classroom for supervision purposes to which she stated that "they are sleeping". I proceeded to ask her to go back into the room because she cannot leave the room unattended. I walked back with her and counted a total of 13 children in the room. I proceeded to check the other classroom for supervision to which she proceeded to walk with me, I once again asked her to stay in her room. Later, some children had to cross the hallway and go into an empty classroom to use the restroom where there was no staff member in the room. The teacher would walk out of her room to peek inside the empty room and check that the children went into the restroom. This complaint is Substantiated as a lack of direct supervision was observed. The section number cited in the inspection report is incorrect. The facility is being issued a Notice of Violation for violating Section 7-33 (e),Ordinance

Of the 12 children, 11 were sleeping or awake in sleeping mats and 1 child was sitting in a high chair with a red house-like toy in front of her. TA was provided that children must not be sat in high chairs at any time other than feeding times

During the course of a complaint inspection the classroom attendance was checked for inspection documentation and the following non- compliance was observed; - some children were marked present even though they were not here (2y/o and 4y/o attendance ) - some children were marked absent even though they were here (2y/o attendance) - some children were marked present in the 3y/o attendance but the children were in the 4y/o room. Child not moved to the 4y/o roster. TA was provided that attendance must be documented accurately
