A complaint was received on 1/11/2023 alleging that on 12/30/22 a child a swollen top lip. A staff told the parent that the child had fell while he was crawling but an incident report was not provided to the parent. A complaint inspection was conducted on 1/13/2023 and statement from the director was received. Based on the director's statement(IS), the child fell and he had a pacifier in his mouth . The teacher picked up the child and held him. Shortly after the parent arrived to pick up the child and an incident report was not provided. Per the director, they did not see the swollen lip because the child had a pacifier in his mouth when the incident occurred. This complaint is substantiated as the child did fall and hit his lip on the floor. The incident report was given to the parent for signature until 1/12/2023
