During a complaint inspection related to diapering of a 1y/o child on 4/15/2026, the school was determined to be out of ratio in the mixed group of 1y/o and 2y/o children in the evening hours after 3PM till 5:00PM. After the 1y/o teacher left the facility at 3pm, the director took the 1y/o children into the 2y/o room to be cared for by the 2y/o teacher and left the premises at 3:30PM. As confirmed by the teacher and director, the teacher was alone with both groups of children. As confirmed by the childrens sign in and sing out logs, the mixed group included a total of 9 children of mixed ages 1 and 2 years of age

Allegation: The child, M.S.(1y/o), is going home everyday, with a soiled diaper. The same amount diapers the parent brings the child with is them amount of diapers he comes home with. The child has a diaper rash on private area. The child was soaking wet yesterday, the parent told the Director about this and they told him it was on CAMERA she said the director admitted he was not changed. FINDINGS: A complaint inspection was conducted on 4/16/2026 and a statement from the teachers and director was received. Per the lead teacher, she left at 3:00PM on Wednesday, 4/15/2026 and the child was dry when he woke up from nap at 2:00PM so she didnt change his diaper. She left the children in their assigned room with the director at 3pm. Per the director, she didnt stay in the room with the 1y/o children. She took them to the 2y/o room instead and left the premises at 3:30PM. The mom picked up the child at around 5:30PM and called at almost 6PM to say that her child was overly soiled. Per the 2y/o teacher, this is the first time they mix the 1y/o group with her 2y/o group. The 1y/o children usually mix with another 1y/o group. She was alone with 11 children, including the 1y/o children. The office staff went into the classroom, checked diapers and left so she was alone with the group. She didnt get a chance to change M.S. when the mom arrived to pick him up around 5:30PM. The mom stood by the door, called him and he went to her. She left and didnt say anything to the teacher. Per corroborating agency who reviewed video footage from the 1y/o classroom, the child in question was not changed all day. The teacher checked the childs diaper once in the morning. She picked him up, put him on the diaper changing table, checked his diaper and put him right back down. Prior to naptime, she washed the childs hands and face and put him down for a nap. After naptime, the teacher checked his diaper again and did not change him. After reviewing the childrens daily schedule, the teachers and corroborating agency statement, It was determined that the child was not changed in the morning, prior to naptime and was not changed for the remainder of the day. The child was picked up at 5:32PM. This complaint is substantiated and a non-compliance for inadequate supervision is being issued because his needs were not met, in that, he was not changed while being cared for at the facility. The child was on the premises TA was provided on routine diapering and following the diapering schedule

Complaint alleges that child A was on the street in the middle of the road. The parent reported the situation and the director checked the cameras and said that that the child was trying to get into office and was able to get out when one of the parents opened the door. The office was very busy. FINDINGS: A complaint inspection was conducted on 3/12/2025 and statements from the director and office administrator was received. Per the director, she stated that the incident happened on 2/27/2025. She was tending to a child that was crying in the office a the time of the incident. It happened near closing time when the child was sent to the office with another child. He went through the security door and into the lobby. There was a parent holding the front door open when another parent approached from behind him. He walked out the door in front of the parent that was going out the door. Another child walked Child A up to the office. Video of incident was reviewed and the following was observed; Date stamp on video: 2/27/2025 at 15:19. Camera view is facing the lobby area and main entrance door. A parent in a black flowery dress is observed opening the entrance door and standing at the door holding it open. Child A comes into camera view walking toward the entrance door, wearing a blue bookbag. He stands in front if the open door. There are two more adults that are in camera view, One appears to be signing a book and the other is walking toward the door behind child A. As a parent walks toward the door, child A walks out in front of that parent and out of the building. This complaint is deemed Substantiated and a non-compliance and notice of violation is being issued
