ALLEGATION: A complaint was reported to CCLE on 9/14/2023 regarding a one year old child that was bitten at school by another child, and the program failed to notify the parent about the incident. FINDINGS: An investigation was conducted on 9/15/2023 which consisted of an interview of the school administrator JW and the staff member SK who were in charge of supervising the infant/one year old classroom. The staff member SK stated that the parent asked the teachers about a bite mark that she found on her child's arm on 9/14/2023, and they responded that 3 days ago the child was bitten by another child while playing with a toy. SK said that the day of the incident, the parent was not notified neither an incident report was documented. An incident report was written on 9/14/2023 however the report was not on file at the time of the inspection. According to the school administrator JW and staff member SK, the original incident report was given to the parent by another staff member that was not present during today's investigation. Based on the information gathered, this allegation is DEEMED SUBSTANTIATED. A notice of violation is being issued for a second time non-compliance on the same standard that failed to comply on a complaint investigation conducted on 9/5/2023. ALLEGATION: A complaint was reported to CCLE on 9/1/2023 regarding two year old child that had an incident at the school. The allegation states that the program failed to notify the parent and did not provide an explanation when the child was picked up from the program. INVESTIGATION: An investigation was conducted on 9/5/2023 which consisted of an interview with the staff member (TT) who was in charge of supervising the two year old classroom. According to TT, she recalls an incident that occurred months ago with the two year old child. She stated that the day of the incident, parents were notified and an incident report was written to document the occurrence. When a copy of the incident report was requested by licensing, the provider stated that they do not have a copy nor the original report because it was given to the parent who never returned it

ALLEGATION: A complaint was reported to CCLE on 1/20/23 regarding an infant child that got red mark in the right eye, and scratch marks around the neck. Parent stated that the facility failed to inform the parent about the child's bruises. INVESTIGATION: An investigation was conducted on 1/23/23 which consisted of interviews with the school administrator and staff in charge of supervising the child. According to the school administrator (JW) she was informed by the staff member (PH) that was in the infant classroom, that she noticed a red dot in the child's eye when she was feeding him. JW asked the staff member PH to be more specific about the red mark and it seemed that the child had a red vessel burst. None of the staff, neither school administrator noticed any marks around the child's neck. They stated that the parent was not informed, and that the program contacts the parents if there is an incident that requires first aid assistance. The program provided a copy of an incident report documenting the red mark in the eye, however the parent was not provided this report when the child was picked up from the program. Based on the information gathered, this allegation is deemed SUBSTANTIATED

Upon arrival, Ms. P opened the door and she walked into the aftercare room. I conducted ratio in the 2s class and saw the teacher sitting at a table near the middle of her classroom. Then, I entered the adjoining mixed group of 3s and 4s. I counted eight children in the classroom but there were no adults in the room. Per the 2y/o teacher, she was watching both her classroom and the mixed group and 3s and 4s. TA was provided that she is in a different room and she cannot supervise both classrooms at the same time. while I spoke to the 2y/o teacher, Ms. P walked up to me and told me she was in charge of that mixed group of 3s and 4s. So, I asked her to stay in the room because the classroom cannot be left unsupervised. I proceeded to enter the school age room and I counted 20 school age children without adult supervision. Then, I entered the infant room and observed 8 children that appeared to be infants and 1y/o sleeping without adult supervision. At this point Ms. Pearl came to the infant classroom and I asked her where all the staff members were and she responded that the were out to lunch. Lack of director supervision was observed in the mixed group of infants and 1y/o, the mixed group of 3y/o and 4y/o, and in the school age room. To come into compliance, the director and Ms. P went into the infant room, the 1y/o teacher went into the school age room and another teacher went into the 3s and 4s class.
